血培养阳性标本污染情况的回顾性分析

摘　　要：目的:分析血培养阳性标本污染情况,为血培养标本质量管理提供参考。方法:回顾性分析2021年1月—2022年12月肇庆市怀集县人民医院送检的9 072例血培养样本阳性病原菌分布情况、污染菌分布情况、污染菌样本来源分布情况。结果:9 072例血培养送检样本中,共检出463例阳性样本,占比5.10%,其中革兰阴性菌以大肠埃希菌、肺炎克雷伯菌、铜绿假单胞菌为主;革兰阳性菌以尿肠球菌、表皮葡萄球菌、金黄色葡萄球菌为主;真菌则以白色念珠菌为主。9 072例血培养送检样本中,共有161例污染样本,占比1.77%。污染菌前三名为表皮葡萄球菌、人葡萄球菌、革兰阳性杆菌。161例血培养污染样本中,内科占比最高,其次分别为重症监护病房、外科、门诊。结论:血培养阳性标本以革兰阴性菌为主,样本污染以表皮葡萄球菌为主,污染标本来源以内科为主,临床需重视血培养标本污染问题,积极采取干预措施,保证血培养标本质量。Objective:To analyze the contamination of positive blood culture specimens and provide reference for the quality management of blood culture specimens.Methods:The distribution of pathogenic bacteria,distribution of contaminated bacteria,and source distribution of contaminated bacteria in 9072 positive blood culture samples submitted for testing by Zhaoqing City Huaiji County People's Hospital from January 2021 to December 2022 were retrospectively analyzed.Results:Among the 9072 blood culture samples submitted,463 positive samples were detected,accounting for 5.10%.The gram-negative bacteria were mainly Escherichia coli,Klebsiella pneumoniae and Pseudomonas aeruginosa.The gram-positive bacteria were mainly Enterococcus urinae,Staphylococcus epidermidis and Staphylococcus aureus.The fungi were mainly Candida albicans.Among the 9072 blood culture samples submitted,there were 161 contaminated samples,accounting for 1.77%.The top three contaminated bacteria were Staphylococcus epidermidis,Staphylococcus hominis and gram-positive bacillus.Among 161 contaminated blood culture samples,internal medicine department accounted for the highest proportion,followed by intensive care unit,surgery department and outpatient department.Conclusion:Blood culture positive specimens are mainly gram-negative bacteria,the sample contamination is mainly Staphylococcus epidermidis,and the source of contaminated specimens is mainly internal medicine department.Clinical attention should be paid to the contamination of blood culture specimens,and active intervention measures should be taken to ensure the quality of blood culture specimens.
