健脾益肾针法联合膀胱功能训练治疗脑卒中后神经源性膀胱临床观察

摘　　要：目的 观察健脾益肾针法联合膀胱功能训练治疗脑卒中后神经源性膀胱(PSNB)的效果。方法 选取2020年1月—2022年6月就诊于厦门市中医院的70例PSNB患者,采用随机数字表法分组,对照组给予膀胱功能训练;治疗组在此训练基础上加用健脾益肾针法治疗。对比2组治疗效果。结果 治疗后,治疗组排尿情况均优于对照组(P<0.05);治疗组残余尿量明显少于对照组(P<0.05);治疗组日常生活能力量表(ADL)评分高于对照组,而汉密尔顿抑郁量表(HAMD)评分低于对照组(P<0.05)。治疗组痊愈率71.43%(25/35)明显优于对照组的34.29%(12/35)(P<0.05);2组均未出现因不良反应终止试验的案例。结论 健脾益肾针法治疗PSNB,能改善排尿情况,提高中风患者的日常生活能力,明显降低卒中后抑郁程度,且安全性高。Objective To observe the effect of invigorating spleen and kidney acupuncture combined with bladder function training on neurogenic bladder after stroke.Methods Seventy patients with neurogenic bladder after stroke who were admitted to Xiamen Hospital of Traditional Chinese Medicine from January 2020 to June 2022 were randomly divided into two groups.The control group was given bladder function training.On the basis of this training,the treatment group was treated with invigorating spleen and kidney acupunc-ture combined with bladder function training.The therapeutic effects of the two groups were compared.Results After treatment,the urination in the treatment group was better than that in the control group(P<0.05).The residual urine volume of treatment group was significantly lower than that of control group(P<0.05).The ADL score of the treatment group was higher than that of the control group,and the Hamilton Depression Scale(HAMD)score was lower than that of the control group(P<0.05).The recovery rate of the treatment group was 71.43%(25/35),which was significantly higher than that of the control group(34.29%,12/35)(P<0.05).There were no cases of trial termination due to adverse reactions in both groups.Conclusion Invigorating spleen and kidney acupuncture in the treatment of neurogenic bladder after stroke can improve urination,improve the daily living ability of stroke patients,significantly reduce the de-gree of depression after stroke,and has high safety.
