强直性脊柱炎患者骶髂关节MRI炎症与外周血淋巴细胞的相关性

摘　　要：目的探讨强直性脊柱炎患者外周血淋巴细胞亚群与骶髂关节MRI炎症之间的关系。方法选取强直性脊柱炎患者42例作为研究组,并选取同时期性别与年龄匹配的健康对照57例作为对照组。比较两组外周血T细胞、B细胞亚群比例。根据骶髂关节MRI将强直性脊柱炎患者分为炎症组和无炎症组,比较两组免疫细胞表型的差异。结果与对照组相比,研究组Th17细胞、Tfh17细胞及Th17/Th2比例显著升高,Th1和Tfh1细胞比例下降。强直性脊柱炎患者中,骶髂关节MRI炎症组幼稚CD4^(+)T细胞比例高于无炎症组,而衰竭CD4^(+)T细胞、功能性CD4^(+)T细胞和效应性记忆CD4^(+)T细胞低于无炎症组。结论强直性脊柱炎患者存在免疫细胞失衡,骶髂关节MRI炎症与CD4^(+)T细胞亚群相关。Objective To investigate the correlation between lymphocyte subsets in peripheral blood and inflammation in MRI of sacroiliac joint in ankylosing spondylitis(AS)patients.Methods Forty-two patients with AS were selected as study group,and 57 healthy controls similar in gender and age were selected as control group.The proportion of T cell and B cell subsets in peripheral blood of these two groups was compared.Patients with AS were divided into inflammatory group and non-inflammatory group according to edema in SIJ MRI,and the differences of immune cell phenotypes were compared.Results Compared with the healthy controls,the proportions of Th17 cells,Tfh17 cells and Th17/Th2 cells in study group were significantly increased,while the proportion of Th1 and Tfh1 cells was decreased.For AS patients,the proportion of naive CD4^(+)T cells in inflammation group was higher when compared with non-inflammation group;while the failed CD4^(+)T cells,functional CD4^(+)T cells and effective memory CD4^(+)T cells were lower in the non-inflammation group.Conclusion There is imbalance of immune cells in patients with AS.CD4^(+)T cell subsets in peripheral blood is correlated to edema of sacroiliac joint.
