摘　　要：目的了解经皮肾镜碎石术患者出院准备度现状及其影响因素。方法采用一般资料调查表、出院准备度量表、出院指导质量量表、综合医院焦虑抑郁量表和社会支持量表对泌尿外科行经皮肾镜取石术的患者进行调查。结果经皮肾镜碎石术患者出院准备度平均得分为(98.43±16.21)分,条目均分为(8.20±1.35)分,各维度条目均分分别为:个人状态(7.64±1.90)分、适应能力(8.35±1.39)分、预期性支持(8.44±1.32)分。出院指导质量、焦虑、抑郁和社会支持是影响患者自我感知出院准备度水平的主要因素(R2=0.611,调整R2=0.604,F=80.168,P<0.001)。结论经皮肾镜碎石术患者出院准备度水平较高,但其个人状态维度得分偏低,需加强患者围术期心理护理和出院指导,以提高患者过渡期自我管理信心。Objective To understand the current status of discharge readiness and its influencing factors in patients undergoing local anesthesia percutaneous nephrolithotomy(PCNL).Methods A survey was conducted on patients undergoing PCNL in urology department using a General Information Questionnaire,Readiness for Hospital Discharge,Quality of Discharge Teaching Scale,Hospital Anxiety and Depression Scale,and Social Support Scale.Results The mean score for discharge readiness in patients undergoing PCNL was 98.43±16.21,with item scores averaging 8.20±1.35.The scores for each dimension were as follows:personal status(7.64±1.9),adaptability(8.35±1.39),and expected support(8.44±1.32).The quality of discharge guidance,anxiety,depression,and social support were identified as the main factors influencing patients′self-perceived level of discharge readiness(R2=0.611,adjusted R2=0.604,F=80.168,P<0.001).Conclusion Patients undergoing local anesthesia percutaneous nephrolithotomy demonstrated a relatively high level of discharge readiness.However,the personal status dimension score was relatively low,indicating a need to enhance perioperative psychological care and discharge guidance to boost patients′confidence in self-management during the transition period.
