摘　　要：目的探讨益生菌联合Orem自护导向疗法对结肠癌患者造口术后的影响。方法选取行结肠癌造口术的患者100例作为研究对象,采用随机数字表法将研究对象分为对照组(n=50)和观察组(n=50)。对照组给予造口术后常规护理干预,观察组则在对照组的基础上给予以益生菌联合Orem自护导向干预疗法。比较干预后两组患者术后康复效果、术后1个月内并发症发生率、干预前后血清白蛋白(ALB)、前白蛋白(PA)、血红蛋白(Hb)及创伤后成长水平以评价干预效果。结果干预后,观察组患者术后首次肛门排气、排便时间、下床活动时间、住院时间均低于对照组,差异均有统计学意义(均P<0.05)。观察组术后1个月内并发症总发生率低于对照组,差异有统计学意义(P<0.05)。干预后两组血清ALB、PA、Hb水平高于干预前,且观察组高于对照组,差异有统计学意义(P<0.05)。干预后观察组患者创伤后成长水平高于对照组,差异有统计学意义(P<0.05)。结论益生菌联合Orem自护导向疗法可促进结肠癌造口术后患者康复进程,降低术后并发症发生率,改善术后营养状况和提升创伤后成长水平。Objective To investigate the influence of probiotics combined with Orem self-care oriented therapy on patients undergoing colostomy after colon cancer surgery.Methods One hundred patients undergoing colostomy due to colon cancer were selected as the research subjects and were randomly divided into control group(n=50)and observation group(n=50)using a random number table.The control group received routine nursing interventions after colostomy,while the observation group received probiotics combined with Orem self-care oriented therapy on the basis of the control group.The post-intervention outcomes,complications within one month after surgery,changes in serum albumin(ALB),prealbumin(PA),hemoglobin(Hb),and the level of post-traumatic growth before and after the intervention were compared to evaluate the intervention′s effectiveness.Results After the intervention,the observation group had significantly lower times for the first anal gas discharge,defecation,time to get out of bed,and length of hospital stay compared to the control group,with all differences being statistically significant(all P<0.05).The total incidence of complications within one month after surgery in the observation group was lower than that in the control group,and the difference was statistically significant(P<0.05).After the intervention,the serum levels of ALB,PA,and Hb in both groups were higher than before the intervention,and the observation group was higher than the control group,with statistically significant differences(all P<0.05).After the intervention,the level of post-traumatic growth in the observation group was higher than that in the control group,and the difference was statistically significant(P<0.05).Conclusion Probiotics combined with Orem self-care oriented therapy can promote the recovery process of patients after colon cancer colostomy,reduce the incidence of postoperative complications,improve postoperative nutritional status,and enhance post-traumatic growth levels.
