摘 要:目的 分析妊娠期糖尿病母亲新生儿脐血膜联蛋白A7水平与脑损伤的相关性。方法 选取60例分娩的妊娠期糖尿病母亲新生儿为研究组,另选取60例分娩的妊娠期正常血糖母亲新生儿为对照组。两组新生儿均抽取脐血,运用酶联免疫吸附法检测脐血膜联蛋白A7水平, 3 d后运用新生儿20项行为神经评分法(NBNA)对新生儿进行神经行为评估;对比两组新生儿脐血膜联蛋白A7水平、NBNA评分以及脑损伤发生率,并运用Pearson相关性分析法分析脐血膜联蛋白A7水平与NBNA评分的相关性。结果 研究组新生儿脐血膜联蛋白A7(27.24±3.18)μg/L高于对照组的(22.95±2.69)μg/L, NBNA评分(36.34±1.27)分低于对照组的(38.25±1.56)分,脑损伤发生率13.33%高于对照组的1.67%(P<0.05)。Pearson相关性分析结果显示,脐血膜联蛋白A7水平与NBNA评分呈负相关性(r=-0.613,P<0.05)。结论 妊娠期糖尿病母亲新生儿脐血膜联蛋白A7水平与脑损伤之间存在一定的关联性,新生儿脐血膜联蛋白A7水平可作为预测新生儿脑损伤的参考指标。Objective To analyze the correlation between umbilical blood annexin A7 level and brain injury in mothers with gestational diabetes mellitus.Methods 60 neonates of mothers with gestational diabetes mellitus were selected as the study group,and 60 neonates of mothers with normal blood glucose during pregnancy were selected as the control group.Umbilical blood was extracted from both groups,and the level of umbilical blood annexin A7 was detected by enzyme-linked immunosorbent assay(ELISA).After 3 d,the neonatal neurobehavior was evaluated by 20-item Neonatal Behavioral Neurological Assessment(NBNA).The umbilical blood annexin A7 level,NBNA score and incidence of brain injury were compared between the two groups,and the correlation between umbilical blood annexin A7 level and NBNA score was analyzed by Pearson correlation analysis.Results The umbilical blood annexin A7 of(27.24±3.18)μg/L in the study group was higher than(22.95±2.69)μg/L in the control group,the NBNA score of(36.34±1.27)points was lower than(38.25±1.56)points in the control group,and the incidence of brain injury of 13.33%was higher than 1.67%in the control group(P<0.05).Pearson correlation analysis showed that there was a negative correlation between umbilical blood annexin A7 level and NBNA score(r=-0.613,P<0.05).Conclusion There is a certain correlation between the level of umbilical blood annexin A7 and brain injury in mothers with gestational diabetes mellitus,and the level of umbilical blood annexin A7 can be used as a reference index to predict neonatal brain injury.
