基于“火郁发之”探讨巨噬细胞极化在烧伤创面愈合中的作用

摘  要:烧伤是临床极为常见的意外伤害之一,巨噬细胞表型极化调控着烧伤创面愈合的全过程,其中巨噬细胞表型长期停留在M1型无法极化为M2型形成慢性炎症是烧伤创面愈合延迟的重要原因。因此针对烧伤创面愈合迟缓,调控巨噬细胞表型极化是重要靶点,也是中医药治疗此病的关键。在中医学中,巨噬细胞表型长期停留在M1型所形成的慢性炎症与“火郁”极为类似,而“火郁”则应以“发之”为治疗方法。因此通过解读“火郁发之”与巨噬细胞极化的病理联系,将现代病理机制与中医理论相结合,以期为中医药调控巨噬细胞极化,促进烧伤创面愈合提供新靶点和方向。Burns are one of the most common and harmful clinical injuries.Macrophage phenotype polarization regulates the whole process of burn wound healing,in which macrophage phenotype stays in M1 type for a long time and cannot polarize to M2 type resulting in chronic inflammation,which is an important reason for the delayed healing of burn wounds.Therefore,to address the delayed healing of burn wounds,regulation of macrophage phenotype polarization is an important point of target,and is also the key to treating this disease in traditional Chinese medicine(TCM).The chronic inflammation formed by macrophage phenotype staying in M1 type for a long time is very similar to “fire stagnation”,which should be treated by “dissipation”.Therefore,in this paper,by deciphering the pathological connection between “fire stagnation requiring dissipation” and macrophage polarization and combining modern pathological mechanisms with TCM theory,we aim to provide new targets and directions for TCM to regulate macrophage polarization for healing of burn wounds.
