加味芍药甘草汤内服外敷治疗急性踝关节扭伤的临床观察

摘　　要：目的 观察加味芍药甘草汤内服外敷治疗急性踝关节扭伤的临床疗效并探讨其作用机制及安全性。方法 62例患者随机分为研究组32例与对照组30例。两组均采用常规PRICE原则基础治疗,对照组同时给予洛索洛芬钠凝胶贴膏外贴,研究组则给予加味芍药甘草汤内服外敷,两组疗程均为14 d。比较两组治疗14 d后疗效,治疗前、治疗14 d后疼痛、踝关节功能、肿胀情况、炎症因子,治疗期间不良反应发生情况。结果 研究组总有效率为93.75%,高于对照组的80.00%(P<0.05)。两组治疗前视觉模拟量表(VAS)评分、肿胀度、Kofoed评分和血清白细胞介素-6(IL-6)、超敏C反应蛋白(hs-CRP)水平比较,差别均不大(均P>0.05)。两组治疗后VAS评分、肿胀度与治疗前比较均降低(均P<0.05),而Kofoed评分则均升高(均P<0.05),且研究组VAS评分、肿胀度、Kofoed评分改善均优于对照组(均P<0.05)。两组治疗后血清IL-6、hs-CRP水平与治疗前比较均降低(均P<0.05),且研究组血清IL-6、hs-CRP水平均低于对照组(均P<0.05)。研究组不良反应发生率9.38%与对照组的6.67%比较,差别不大(P>0.05)。结论 加味芍药甘草汤内服外敷治疗急性踝关节扭伤患者,可减轻患者踝关节疼痛、肿胀情况,改善踝关节功能,降低患者血清炎症因子水平,安全性良好。Objective:To observe the clinical efficacy on the treatment of acute ankle sprain and safety with oral and external application of Modified Shaoyao Gancao Decoction.Methods:A total of 62 patients were randomly divided into two groups,with 32 cases in the study group and 30 cases in the control group.The control group was treated with loxoprofen sodium gel plaster,and the study group was treated with Modified Shaoyao Gancao Decoction for oral and external application.Both groups were treated for 14 days.The therapeutic effects,pain,ankle joint function,swelling,inflammatory factors before and after 14 days of treatment,and occurrence of adverse reactions during the treatment period were compared between the two groups.Results:After 14 days of treatment,the total effective rate in the study group was 93.75%,which was significantly higher than 80.00%of the control group(P<0.05).Compared with before treatment,after 14 days of treatment,the Kofoed ankle joint function score increased in both groups,with a higher score in the study group(P<0.05).The visual analog scale(VAS)scores,ankle joint circumference,interleukin-6,and high-sensitivity C-reactive protein levels decreased in both groups,with a lower level in the study group(P<0.05).During the treatment period,the incidence of adverse reactions in the study group and the control group was 9.38%and 6.67%,respectively,with no significant difference(P>0.05).Conclusion:Oral and external application of Modified Shaoyao Gancao Decoction has significant efficacy in the treatment of acute ankle sprain,which can alleviate ankle joint pain and swelling,improve ankle joint function,and reduce the level of serum inflammatory factors in patients with good safety.
