摘 要:目的探索早发血管衰老(EVA)对左房扩大的相关性。方法选取2010年5月至2022年7月华中科技大学同济医学院附属同济医院同济队列人群554例,所有人均接受病史采集、辅助检查、血生化检验。根据左房是否扩大分为对照组与研究组,使用t检验研究两组间一般情况差异。应用Spearman相关性分析心脏结构功能与动脉硬化指标之间的相关性。将左房扩大作为事件,二元Logistic分析影响因素,拟合受试者操作特性曲线(ROC),判断包括EVA在内的联合诊断左房扩大的有效性。结果研究组男性占比92.6%,平均年龄(47.38±6.77)岁,心率(69.76±15.51)bpm,收缩压为(126.40±26.50)mmHg、舒张压(95.82±21.43)mmHg、平均动脉压为(106.01±22.95)mmHg均显著高于对照组(t=-2.977、-3.179、-3.131,P<0.05);研究组baPWV(1379.38±282.43)cm/s,显著高于对照组[(1296.98±200.85)cm/s,t=-3.153,P<0.01],组内EVA人群比例占7.4%,高于对照组2.4%(t=-2.095,P<0.05)。Spearman相关性结果示baPWV、收缩压、舒张压、平均动脉压与左房内径显著相关(r=0.191、0.360、0.341、0.354,P<0.05)。校正性别、年龄、心率的二元Logistic模型中,EVA是左房扩大的独立危险因素。联合诊断ROC具有诊断意义[曲线下面积(AUC)=0.733,P<0.01]。结论EVA与左房扩大显著相关,EVA是左房扩大的独立危险因素。Objective To explore the relationship between early vascular ageing(EVA)and left atrial enlargement.Methods We selected 554 individuals from the Tongji cohort of Tongji Hospital affiliated to Tongji Medical College,Huazhong University of Science and Technology from May 2010 to July 2022,all of whom underwent medical history collection,auxiliary examinations,and blood biochemical tests.Divide into a control group and a study group based on whether the left atrium is enlarged,and use t-test to study the general differences between the two groups.Apply Spearman correlation analysis to analyze the correlation between cardiac structure and function,arterial stiffness indicators.Using left atrial enlargement as an event,binary logistic analysis was conducted to identify the influencing factors.Fit the receiver operating characteristic curve(ROC),and evaluate the effectiveness of joint diagnosis of left atrial enlargement,including EVA.Results The proportion of males in the study group was 92.6%,with an average age of(47.38±6.77)years,heart rate of(69.76±15.51)bpm,systolic blood pressure of(126.40±26.50)mmHg,diastolic blood pressure of(95.82±21.43)mmHg,and average arterial pressure of(106.01±22.95)mmHg,all of which were significantly higher than those in the control group(t=-2.977,-3.179,-3.131,P<0.05);The study group’s baPWV was[(1379.38±282.43)cm/s],significantly higher than the control group’s[(1296.98±200.85)cm/s,t=-3.153,P<0.01],and the proportion of EVA population in the group was 7.4%,higher than the control group’s 2.4%(t=-2.095,P<0.05).The Spearman correlation results showed a significant correlation between baPWV,systolic blood pressure,diastolic blood pressure,mean arterial pressure,and left atrial diameter(r=0.191,0.360,0.341,0.354,P<0.05).EVA is an independent risk factor for left atrial enlargement in a binary logistic model that corrects for gender,age,and heart rate.Joint diagnosis of ROC has diagnostic significance[area under curve(AUC)=0.733,P<0.01].Conclusion EVA is significantly correlat
