摘 要:目的研究白蛋白/球蛋白比率(albumin-globulin ratio,AGR)预测糖尿病肾病血液透析患者死亡率的价值。方法选取2010年1月1日至2018年1月1日到内蒙古科技大学包头医学院第一附属医院肾内科就诊的规律随访且病情稳定的糖尿病肾病血液透析患者,收集患者的基线临床资料,包括一般资料、实验室检查、合并症、用药情况。根据AGR分为AGR≥1.2组及AGR<1.2组。所有患者随访至2022年1月1日。结果90例糖尿病肾病血液透析患者纳入研究。平均随访时间为(4.57±3.16)年。两组患者心血管疾病、空腹血糖、糖化血红蛋白、白蛋白、总蛋白、血红蛋白、血肌酐差异有统计学意义(P<0.05)。高AGR组和低AGR组分别有8例(17.7%)和6例(37.5%)患者死亡。单变量Cox和多因素回归分析结果显示,低AGR与全因死亡率和心血管事件发生率显著相关,并且是糖尿病肾病血液透析患者死亡的独立预测因子。结论AGR可以预测糖尿病肾病血液透析患者的死亡率。Objective To investigate whether albumin-globulin ratio(AGR)can predict mortality in hemodialysis patients with diabetic nephropathy.Methods A total of 90 hemodialysis patients with diabetic nephropathy in Department of Nephrology of The First Affiliated Hospital of Baotou Medical College were selected and followed up regularly from January 1th,2010 to January 1th,2018.Methods The baseline clinical data including general data,laboratory examination,comorbidities and medications were collected.The patients were divided into 2 groups according to the ratio of albumin/globulin(AGR):AGR≥1.2 and AGR<1.2.All the patients were followed up until January 1th,2022.Results Ninety hemodialysis patients with diabetic nephropathy were included in the study.The mean follow-up time was(4.57±3.16)years.There were significant differences in cardiovascular diseases,albumin,total protein,hemoglobin and serum creatinine between the two groups(P<0.05).Eight(17.7%)and six(37.5%)patients in the high-AGR group or low-AGR group died,respectively.According to univariate Cox and multivariate regression analysis,low AGR was significantly associated with all-cause mortality and cardiovascular events and was an independent predictor of death in hemodialysis patients with diabetic nephropathy.Conclusion AGR can predict mortality in hemodialysis patients with diabetic nephropathy.
