归脾二仙汤辅助治疗对围绝经期抑郁症(脾肾两虚型)患者卵巢功能、神经内分泌及睡眠质量的影响

摘　　要：目的:探讨归脾二仙汤辅助治疗对围绝经期抑郁症(脾肾两虚型)患者的影响。方法:选取2021年1月至2023年1月山西省人民医院中医内科收治的围绝经期抑郁症患者120例作为研究对象,按照数字编号信封法随机分为观察组和对照组,每组60例。观察组给予归脾二仙汤联合西药治疗,对照组给予常规西药治疗,比较2组临床疗效,比较2组治疗前、治疗后3个月、6个月的更年期症状严重程度(Kupperman评分量表),记录2组治疗前后的卵巢功能[卵泡刺激素(FSH)、黄体生成素(LH)和雌二醇(E_(2))]、神经内分泌功能[促甲状腺激素(TSH)、四碘甲状腺原氨酸(T4)和三碘甲状腺原氨酸(T3)]、睡眠质量和认知功能,并进行比较。结果:治疗后E_(2)、T4、T3水平和MMSE评分高于治疗前(均P<0.05),在临床有效率、E_(2)、T4、T3水平和MMSE评分比较上,观察组高于对照组(均P<0.05);治疗后FSH、LH、TSH水平和PSQI评分低于治疗前,在FSH、LH、TSH水平和PSQI评分比较上,观察组低于对照组(均P<0.05);治疗3个月、6个月后的Kupperman评分低于治疗前,且观察组低于对照组(均P<0.05)。结论:归脾二仙汤联合常规西药治疗围绝经期抑郁症的效果更好,可减轻更年期症状,改善卵巢功能、神经内分泌功能、睡眠质量和认知功能,可在临床推广应用。Objective:To explore the effect of Guipi Erxian Decoction as an auxiliary treatment on patients with perimenopausal depression(spleen kidney deficiency type).Methods:A total of 120 patients with perimenopausal depression who visited our hospital from January 2021 to January 2023 were randomly selected and divided into an observation group and a control group using the number numbered envelope method.Each group had 60 cases.The observation group was treated with Guipi Erxian Decoction combined with western medicine,while the control group was treated with conventional western medicine.Compare the clinical efficacy of two groups,compare the severity of menopausal symptoms(Kupperman score scale)before treatment,3 months after treatment,and 6 months after treatment.The ovarian function[follicle stimulating hormone(FSH),Luteinizing hormone(LH)and estradiol(E_(2))],neuroendocrine function[Thyroid stimulating hormone(TSH),tetraiodothyronine(T4)and Triiodothyronine(T3)],sleep quality[Pittsburgh Sleep Quality Index(PSQI)]and cognitive function(MMSE)were compared.Results:In terms of clinical efficacy,E_(2),T4,T3 levels,and MMSE scores,the observation group was higher than the control group(P s<0.05),and the E_(2),T4,T3 levels,and MMSE scores after treatment were higher than before treatment(P s<0.05).In terms of FSH,LH,TSH levels and PSQI scores,the observation group was lower than the control group(P s<0.05),and the FSH,LH,TSH levels and PSQI scores after treatment were lower than before treatment(P s<0.05).After 3 and 6 months of treatment,the Kupperman score was lower than before treatment(P<0.05),and the observation group was loeer than the control group(P<0.05).Conclusion:The combination of Guipi Erxian Decoction and conventional western medicine has a better effect on the treatment of perimenopausal depression,can alleviate menopausal symptoms,improve ovarian function,neuroendocrine function,sleep quality,and cognitive function,and can be promoted and applied in clinical practice.
