摘  要:前交叉韧带重建术(anterior cruciate ligament reconstruction,ACLR)是前交叉韧带撕裂的有效治疗方式。尽管手术治疗发挥了积极效应,但多种因素影响ACLR术后疗效,其中股骨隧道位置定位的差异是造成手术失败的主要原因之一。研究综述了关节镜下ACLR股骨隧道定位技术的演变,包括早期的等长重建、经典的解剖重建以及最新提出的I.D.E.A.L.重建,并探讨了不同重建方式的疗效优劣势。研究认为,无论是等长重建还是解剖重建,股骨隧道内口定位在前内侧束位置具有更良好的疗效,但解剖重建比等长重建能够获得更好的移植物成熟度,而I.D.E.A.L.重建的疗效还需要长期临床研究以及机制验证,以提供证据支持。Anterior cruciate ligament reconstruction(ACLR)is an effective treatment for anterior cruciate ligament tears.Despite the positive effect of surgical treatment,several factors influence the postoperative efficacy of ACLR,among which the discrepancy in the positioning of the femoral tunnel is one of the main reasons for the failure of the operation.This study reviewed the evolution of arthroscopic ACLR reconstructive femoral tunnel position techniques over the past decades including early isometric reconstruction,classical anatomic reconstruction,and the newly proposed I.D.E.A.L.reconstruction.The advantages and disadvantages of postoperative efficacy of different reconstructive approaches were discussed.It was concluded that either isometric or anatomic reconstruction,femoral tunnel position in the anteromedial bundle position has better efficacy,but anatomic reconstruction results in better graft maturity than isometric reconstruction,while long-term clinical studies as well as mechanistic validation are needed to provide evidence to support the efficacy of I.D.E.A.L.reconstruction.
