摘 要:目的比较两种切口钢板内固定治疗胫骨平台骨折的疗效。方法将108例胫骨平台骨折患者按照切口不同分为观察组(采用微创切口钢板内固定治疗,54例)和对照组(采用传统切开复位钢板内固定治疗,54例)。记录两组切口长度、住院时间、骨痂形成时间、骨折愈合时间、开始部分负重行走时间、并发症发生情况、膝关节活动度、膝关节功能优良率。结果患者均获得随访,时间12~18个月。切口长度、住院时间、骨痂形成时间、骨折愈合时间、开始部分负重行走时间观察组均短(早)于对照组(P<0.05)。术后6个月,膝关节功能优良率观察组高于对照组(P<0.01),膝关节屈曲、伸直活动度观察组均优于对照组(P<0.05)。术后并发症发生率观察组低于对照组(P<0.05)。结论与传统切开复位钢板内固定相比,微创切口钢板内固定治疗胫骨平台骨折具有创伤小、住院时间短、并发症发生率低的优点,更利于骨折愈合、患者早期功能锻炼及膝关节功能的恢复。Objective To compare the effects of plate internal fixation for tibial plateau fractures via two kinds of incisions.Methods According to different incisions,108 patients with tibial plateau fractures were divided into observation group(54 cases were treated with minimally invasive incision and plate internal fixation)and control group(54 cases were treated with traditional open reduction and plate internal fixation).The incision length,hospital stay time,callus formation time,fracture healing time,start time of walking with partial weight-bearing,complications situation,range of motion of knee joint,and the excellent-good rate of knee joint function were recorded in both groups.Results All patients were followed up for 12~18 months.The incision length,hospital stay time,callus formation time,start time of walking with partial weight-bearing in the observation group were all shorter(earlier)than those in the control group(P<0.05).Six months after surgery,the excellent-good rate of knee joint function in the observation group was higher than that in the control group(P<0.01),and the range of motion of knee joint with flexion and extension in the observation group were both better than those in the control group(P<0.05).The incidence rate of complications in the observation group was lower than that in the control group(P<0.05).Conclusions Compared with traditional open reduction and plate internal fixation,minimally invasive incision and plate internal fixation for the treatment of tibial plateau fractures have the advantages of less trauma,shorter hospital stay time,and lower incidence rate of complications,which are more conducive to fracture healing,early functional exercise of patients and recovery of knee joint function.
