妊娠期糖尿病患者子痫前期发病的相关临床分析

摘　　要：目的妊娠期糖尿病患者子痫前期发病的相关临床分析。方法选取2023年1月—2023年9月三明市第二医院收治的100例妊娠期糖尿病孕妇为研究对象,根据其是否发生子痫前期进行分组。对照组50例妊娠期糖尿病孕妇未发生子痫前期,观察组50例妊娠期糖尿病孕妇发生子痫前期。对比两组患者可溶性fms样酪氨酸激酶-1(soluble fms-like tyrosine kinase 1,sFlt-1)、胎盘生长因子(placental growth factor,PlGF),分析糖尿病孕妇子痫前期严重程度与sFlt-1、PlGF以及二者比值的关系。结果观察组孕妇sFlt-1、sFlt-1/PlGF水平高于对照组,差异有统计学意义(P<0.05);观察组孕妇PlGF水平低于对照组,差异有统计学意义(P<0.05);观察组孕妇中,重度子痫前期患者sFlt-1、sFlt-1/PlGF水平更高,PlGF水平更低,差异有统计学意义(P<0.05)。Pearson相关性分析结果显示,糖尿病孕妇子痫前期的严重程度与sFlt-1、sFlt-1/PlGF成正相关,与PlGF呈负相关(r=0.514、-0.209,P<0.05)。结论妊娠期糖尿病合并子痫前期孕妇sFlt-1及其二者比值均显著高于妊娠期糖尿病孕妇、PlGF的值低于妊娠期糖尿病,且糖尿病孕妇子痫前期的严重程度与sFlt-1、sFlt-1/PlGF成正相关,与PlGF呈负相关,对于患有妊娠期糖尿病孕妇应强化孕期管理,降低子痫前期发生风险,改善妊娠结局。Objective To analyze the clinical analysis related to the onset of preeclampsia in patients with gestational diabetes mellitus.Methods A total of 100 pregnant women with gestational diabetes admitted to the Second Hospital of Sanming City from January 2023 to September 2023 were selected as the study objects,and they were grouped ac-cording to whether they had preeclampsia.In the control group,50 pregnant women with gestational diabetes did not develop preeclampsia,and in the observation group,50 pregnant women with gestational diabetes did.soluble fms-like tyrosine kinase 1(sFlt-1),placental growth factor(PlGF)and soluble fms-like Tyrosine kinase 1(sFLT-1)were compared between two groups.To analyze the relationship between the severity of preeclampsia and sFlt-1,PlGF and their ratio in diabetic pregnant women.Results The levels of sFlt-1 and sFlt-1/PlGF in observation group were higher than those in control group,and the differences were statistically significant(P<0.05).The PlGF level in the observa-tion group was lower than that in the control group,and the difference was statistically significant(P<0.05).In the ob-servation group,sFlt-1 and sFlt-1/PlGF levels were higher and PlGF levels were lower in patients with severe pre-eclampsia,and the differences were statistically significant(P<0.05).Pearson correlation analysis showed that the se-verity of preeclampsia in diabetic pregnant women was positively correlated with sFlt-1,sFlt-1/PlGF,and negatively correlated with PlGF(r=0.514,-0.209,P<0.05).Conclusion The values of sFlt-1 and its ratio were significantly higher than those of pregnant women with gestational diabetes mellitus and lower than those of pregnant women with gestational diabetes mellitus,and the values of PlGF were lower than those of pregnant women with gestational diabe-tes mellitus,and the severity of preeclampsia in pregnant women with diabetes mellitus was positively correlated with sFlt-1,sFlt-1/PlGF and negatively correlated with PlGF.Pregnant women with gestational diabetes mellitus shou
