摘　　要：目的探讨针对不稳定型心绞痛(unstable angina pectoris,UAP)合并上消化道出血(upper gastrointestinal bleeding,UGIB)患者实行无痛胃镜检查,相较于普通胃镜,两者的临床效果差异。方法选取2022年1月—2023年6月滨州市中心医院收治的72例UAP合并UGIB患者为研究对象,随机数表法分为对照组与观察组,各36例。对照组应用普通胃镜检查,观察组实施无痛胃镜。比较两组生命体征、心电图改变、不良反应发生率、检查效果和焦虑抑郁程度。结果检查中,观察组收缩压、心率均低于对照组,差异有统计学意义(P<0.05)。观察组ST段压低、心律失常发生率分别为11.11%、5.56%,均低于对照的组38.89%、25.00%,差异有统计学意义(χ^(2)=4.938,5.257,P<0.05)。观察组检查优良率高于对照组,差异有统计学意义(P<0.05)。观察组不良反应发生率低于对照组,差异有统计学意义(P<0.05)。检查后,观察组焦虑自评量表、抑郁自评量表评分低于对照组,差异有统计学意义(P<0.05)。结论UAP合并UGIB患者应用无痛胃镜检查,有利于稳定患者生命体征,减少检查不良反应发生率,保证检查的安全性。Objective To investigate the differences in clinical outcomes of painless gastroscopy compared with ordinary gastroscopy in patients with unstable angina pectoris(UAP)combined with upper gastrointestinal bleeding(UGIB).Methods A total of 72 patients with UAP combined with UGIB admitted to Binzhou Central Hospital from January 2022 to June 2023 were selected as the research object and divided into control group and observation group by random number table method,with 36 cases in each group.The control group received general gastroscopy,and the observation group received painless gastroscopy.The vital signs,ECG changes,incidence of adverse reactions,examination effect and anxiety and depression were compared between the two groups.Results During the examination,the systolic blood pressure and heart rate of the observation group were lower than those of the control group,and the differences were statistically significant(P<0.05).The incidence of ST segment depression and arrhythmia in the observation group were 11.11% and 5.56%,respectively,which were lower than 38.89% and 25.00% in the control group,and the differences were statistically significant(χ^(2)=4.938,5.257,P<0.05).The examination excellent and good rate of the observation group was higher than that of the control group,and the difference was statistically significant(P<0.05).The incidence of adverse reactions in observation group was lower than that in control group,and the difference was statistically significant(P<0.05).After examination,the scores of self-rating anxiety scale and self-rating depression scale in the observation group were lower than those in the control group,and the difference was statistically significant(P<0.05).Conclusion The application of painless gastroscopy in patients with UAP combined with UGIB is conducive to stabilizing patients'vital signs,reducing the incidence of adverse reactions of the examination,and ensuring the safety of the examination.
