摘　　要：目的探讨维持性血液透析患者KT/V水平与心脏结构和功能的关系。方法回顾性分析2023年1—4月汕头市中心医院进行维持性血液透析患者194例,根据患者KT/V水平分为A组(108例,KT/V<1.4)和B组(86例,1.4≤KT/V<1.8)。比较两组年龄、透龄、血红蛋白、血白蛋白、尿酸、同型半胱氨酸、尿素氮、肌酐、收缩压、舒张压、左室射血分数、左房收缩末期内径、左室舒张末内径、室间隔厚度、左心室后壁厚度,舒张早期和舒张晚期二尖瓣口最大血流速度之比(E/A)的差别。采用Pearson方法分析各个指标与KT/V相关性。结果与B组相比,A组左房直径(left anterior descending,LAD)、左室后壁厚度(left ventricular posterior wall thickness,LVPWD)明显增大增厚,差异有统计学意义(P<0.05);两组其他指标比较,差异无统计学意义(P>0.05)。相关性分析显示KT/V与LAD及LVPWD呈负相关(r=-0.233,-0.246,P<0.05)。结论KT/V水平与LAD、LVPWD呈负相关,KT/V水平与维持性血透患者心脏结构密切相关。Objective To investigate the relationship between KT/V levels and cardiac structure and function in maintenance hemodialysis patients.Methods A retrospective analysis was performed on 194 patients undergoing maintenance hemodialysis in Shantou Central Hospital from January to April 2023.According to KT/V level,the patients were divided into group A(108 cases,KT/V<1.4)and group B(86 cases,1.4≤KT/V<1.8).The differences of age,age of transmission,hemoglobin,blood albumin,uric acid,homocysteine,urea nitrogen,creatinine,systolic blood pressure,diastolic blood pressure,left ventricular ejection fraction,left atrial end-systolic diameter,left ventricular enddiastolic diameter,interventricular septum thickness,posterior left ventricular wall thickness,and the ratio of mitral orifice maximum flow velocity(E/A)between the two groups were compared.Pearson method was used to analyze the correlation between each index and KT/V.Results Compared with group B,the left anterior descending(LAD)and left ventricular posterior wall thickness(LVPWD)in group A significantly increased and thickened,the difference was statistically significant(P<0.05).There was no statistically significant difference in other indexes between the two groups(P>0.05).Correlation analysis showed that KT/V was negatively correlated with LAD and LVPWD(r=-0.233,-0.246,P<0.05).Conclusion KT/V level was negatively correlated with LAD and LVPWD,and KT/V level was closely related to the cardiac structure of maintenance hemodialysis patients.
