探讨回收式自体输血在出血高危剖宫产术中的应用效果

摘　　要：目的对剖宫产术中应用回收式自体输血的效果进行探讨分析。方法选取2020年1月—2023年6月东莞市妇幼保健院收治的120例行剖宫产术的产妇为研究对象,按随机数表法分为异体血输血组(allogeneic blood transfusion,ABT)和自体血回输组(intra-operative cell salvage,ICS),每组60例,比较两组患者自体血回输量及手术前后主要凝血功能指标、手术后输血不良反应的发生率。结果术后自体血回输组的活化部分凝血活酶时间(26.24±4.22)s、凝血酶原时间(11.06±1.43)s、凝血酶时间(17.82±2.51)s、血浆纤维蛋白原(2.02±0.58)g/L均低于异体血回输组,差异有统计学意义(P<0.05);异体血输血组手术后输血不良反应的发生率(15.00%)明显高于自体血回输组,差异有统计学意义(χ^(2)=7.688,P<0.05)。结论对剖宫产术中应用回收式自体输血的效果确切,同时具备较高的安全性。Objective To discuss and analyze the effect of recovery autotransfusion in cesarean section.Methods A total of 120 pregnant women who underwent cesarean section in Dongguan Maternal and Child Health Hospital from January 2020 to June 2023 were selected as the research objects.According to the random number table method,they were divided into allogeneic blood transfusion group(ABT)and intra-operative cell salvage group(ICS),with 60 cases in each group.The amount of autologous blood transfusion,the main coagulation function indexes before and after operation,and the incidence of blood transfusion adverse reaction after operation were compared between the two groups.Results The activated partial thromboplastin time(26.24±4.22)s,prothrombin time(11.06±1.43)s,thrombin time(17.82±2.51)s,plasma fibrinogen(2.02±0.58)g/L in the autologous blood reinfusion group were lower than those in the allogeneic blood reinfusion group,and the differences were statistically significant(P<0.05).The incidence of postoperative blood transfusion adverse reaction in the allogeneic blood transfusion group(15.00%)was significantly higher than that in the autologous blood transfusion group,and the difference was statistically significant(χ^(2)=7.688,P<0.05).Conclusion The application of recovered autotransfusion in cesarean section has the exact effect and high safety.
