摘　　要：近年来,国家卫生健康委员会、国家疾病预防控制局积极推进医防协同、医防融合工作,推动建立以专业公共卫生机构为骨干、以医疗机构为依托、以基层医疗卫生机构为网底的疾病预防控制体系。南通市通过出台优化疾病预防控制体系实施方案、扩充医防协同工作专业人员、筹建慢性病管理医防协作联盟等探索与实践,创新建立医防协同模式,推动形成贴近群众健康服务需求的、可复制可推广的成功经验,努力为人民群众提供全方位、全周期的健康服务。文章对南通市创新医防协同的背景进行阐述,通过分析南通市医防协同工作开展现状以及在实践探索过程中出现的问题,提出解决措施和优化方案,为下一步工作开展提供思路。In recent years,the National Health Commission and the National Bureau of Disease Control and prevention have ac⁃tively promoted medical-prevention coordination and integration,and promoted the establishment of professional public health institu⁃tions as the backbone and medical institutions as the backing,the system of disease prevention and control is based on the network of primary medical and health institutions.The city of Nantong has made great achievements in the exploration and practice of im⁃proving the implementation of the disease prevention and control system,expanding the number of professionals in the medical and preventive cooperation,and preparing for the establishment of a medical and preventive cooperation alliance for chronic disease man⁃agement,it will create a new model of medical cooperation and prevention,promote the formation of successful experiences that are close to the needs of the people and can be replicated and replicated,and strive to provide all-round and full-cycle health services for the people.It describes the background of Nantong’s innovative medical and preventive cooperation,and proposes solutions and optimization plans based on the analysis of the current situation of the implementation of medical and preventive cooperation in Nantong and the problems arising from the practical exploration process,for the next work to provide ideas.
