高通透析器配合改良预冲方式在无肝素透析患者中的应用

摘  要:目的:探讨高通透析器配合膜加温预冲方式在无肝素透析患者中的应用效果。方法:选取在某三甲医院血液净化中心行无肝素血液透析的60例患者作为研究对象,采用自身对照法,前3次透析采用传统预冲方法的患者设为对照组,后3次透析采用改良后的预冲方法的患者设为观察组。比较两种预冲方法在无肝素透析过程中的有效透析治疗时间、更换管道次数、透析器或管路凝血情况以及患者满意度。结果:与对照组相比,观察组采用高通透析器配合膜加温后的预冲方法进行无肝素透析时,有效透析治疗时间更长,更换管道次数更少,透析器或管路凝血程度更轻,患者满意度更高,以上差异均有统计学意义(P<0.05)。结论:高通透析器配合膜加温预冲方法可降低无肝素透析凝血发生率,降低医疗成本,提高患者透析充分性和满意度,方便临床使用。Objective:To investigate the application effects of high⁃flux dialyzer combined with amended membrane heating preflush scheme in heparin⁃free dialysis patients.Methods:60 patients undergoing heparin⁃free blood dialysis in a tertiary hospital′s blood purification center were selected as the research subjects.Self⁃comparison method was adopted.The patients underwent traditional preflushing method for the first 3 times of dialysis(set as the control group)and improved preflushing method for the last 3 times of dialysis(set as the observation group).Effective dialysis treatment time,pipe replacement times,dialyzer or line clotting,and patient satisfaction were compared between the two preflushing methods during heparin⁃free dialysis.Results:The high⁃flux dialyzer with modified membrane heating preflushing method for heparin⁃free dialysis had longer effective dialysis treatment time,fewer pipe replacement times,less coagulation degree of dialyzer or pipe,and higher patient satisfaction.Conclusion:The high⁃flux dialyzer with modified membrane heating preflushing method can reduce the incidence of coagulation in heparin⁃free dialysis,reduce medical costs,improve the adequacy and satisfaction of dialysis in patients,thus it is worthy of its clinical use and popularization.
