摘  要:目的:探讨床旁B超动态定位技术在极早产儿经外周静脉置入中心静脉导管(PICC)中的应用价值。方法:选取新生儿重症监护室(NICU)住院期间拟留置PICC的极早产儿160例,其中的80例作为对照组,采用盲穿置管方法+常规护理,其余的80例作为干预组,采用超声引导置管、床旁B超动态定位监测+常规护理。比较两组PICC导管的继发性异位率、人工矫正率、重新置管率。结果:干预组PICC导管继发性异位率、重新置管率均低于对照组,差异有统计学意义(P<0.05)。干预组人工矫正率高于对照组(P<0.05)。结论:床旁B超动态定位技术能有效降低极早产儿PICC导管继发性异位率,提高人工矫正成功率。Objective:To explore the application value of bedside B⁃mode ultrasound dynamic localization technique in the peripherally inserted central catheter(PICC)for extremely preterm infants.Methods:A total of 160 extremely preterm infants who were planned to have PICC placement during their stay in the neonatal intensive care unit(NICU)were selected.Among them,80 infants were assigned to the control group and received blind puncture catheterization method combined with routine care,while the remaining 80 infants were assigned to the intervention group and underwent ultrasound⁃guided catheterization using bedside B⁃mode ultrasound dynamic localization technique along with routine care.The secondary malposition rate,manual correction rate,and re⁃catheterization rate of PICC were compared between the two groups.Results:The intervention group had significantly lower rates of secondary malposition and re⁃catheterization of PICC compared to the control group,and the difference was statistically significant(P<0.05).The manual correction rate was higher in the intervention group than in the control group(P<0.05).Conclusion:Bedside B⁃mode ultrasound dynamic localization technique can effectively reduce the rate of secondary malposition of PICC in extremely preterm infants and improve the success rate of manual correction.
