摘　　要：目的探讨子宫内膜炎对宫腔粘连术后辅助生殖患者妊娠结局的影响。方法回归性分析2020年7月至2021年6月于江西省妇幼保健院日间手术中心行宫腔粘连分离术治疗的160例中重度宫腔粘连不孕症患者的临床资料。收集中重度宫腔粘连不孕症患者的临床资料,包括年龄、体重指数(BMI)、流产次数、粘连程度及是否合并慢性子宫内膜炎等,以是否可见孕囊分为临床妊娠患者与未临床妊娠患者,单因素与多因素Logistic回归分析宫腔粘连术后辅助生殖患者妊娠结局的影响因素。结果单因素分析结果显示,年龄、流产次数、合并慢性子宫内膜炎发生率为中重度宫腔粘连不孕症患者妊娠结局的影响因素(P<0.05);临床妊娠患者与未临床妊娠患者BMI、粘连程度比较差异无统计学意义。多因素Logistic回归分析结果显示,年龄、流产次数、合并慢性子宫内膜炎均为中重度宫腔粘连不孕症患者妊娠结局的独立危险因素(P<0.05)。非慢性子宫内膜炎患者术后胚胎种植率、临床妊娠率、活产率均高于慢性子宫内膜炎患者,流产率低于慢性子宫内膜炎患者,差异有统计学意义(P<0.05)。结论年龄、流产次数、合并慢性子宫内膜炎均为中重度宫腔粘连不孕症患者妊娠结局的危险因素,临床给予针对性干预措施,进而改善患者预后,以改善患者辅助生殖后的妊娠结局。Objective To investigate the effect of endometritis on pregnancy outcome in patients with assisted reproduction after intrauterine ad-hesions.Methods Regression analysis was performed on the clinical data of 160 patients with moderate to severe intrauterine adhesion infertility who were treated with intrauterine adhesion separation at the Day Surgery Center of Jiangxi Maternal and Child Health Hospital from July 2020 to June 2021.The clinical data of infertility patients with moderate to severe intrauterine adhesions were collected,including age,body mass index(BMI),number of abortions,degree of adhesion and whether chronic endometritis was combined in time,the patients were divided into clinical preg-nancy patients and non-clinical pregnancy patients according to whether the gestational sac was visible,univariate and multivariate Logistic regres-sion analysis were used to analyze the influencing factors of pregnancy outcome in patients with assisted reproduction after intrauterine adhesions.Results Univariate analysis showed that age,number of abortions,and incidence of chronic endometritis were the influencing factors of pregnancy outcome in infertility patients with moderate to severe intrauterine adhesions(P<0.05);there was no significant difference in BMI and adhesion de-gree between clinical pregnancy patients and non-clinical pregnancy patients.Multivariate Logistic regression analysis showed that age,number of abortions,and chronic endometritis were independent risk factors for pregnancy outcomes in infertile patients with moderate to severe intrauterine ad-hesions(P<0.05).The postoperative embryo implantation rate,clinical pregnancy rate and live birth rate of patients with non-chronic endometritis were higher than those of patients with chronic endometritis,and the abortion rate was lower than that of patients with chronic endometritis,the dif-ferences were statistically significant(P<0.05).Conclusion Age,number of abortions,and chronic endometritis are all risk factors for pregnancy outcomes in patient
