血清降钙素原检测在慢性重型乙型肝炎合并细菌感染患者中的诊断价值

摘　　要：目的探讨血清降钙素原(PCT)检测在慢性重型乙型肝炎合并细菌感染患者中的诊断价值。方法选取2019年2月至2022年1月于湖北省荆州市中医医院诊治的80例重型乙型肝炎患者的临床资料,按照是否合并细菌感染分为感染组与无感染组,各40例。比较两组治疗前血清降钙素原(PCT)、C反应蛋白(CRP)及红细胞沉降率(ESR)和白细胞计数(WBC)水平,分析PCT、CRP、WBC、ESR在慢性重型乙型肝炎是否合并细菌感染中的诊断价值。结果治疗前,感染组血清PCT、CRP、WBC水平及ESR均高于无感染组,差异有统计学意义(P<0.05)。PCT、CRP、WBC、ESR、PCT联合CRP、PCT联合WBC及PCT联合ESR灵敏度、准确率比较差异有统计学意义(P<0.05);PCT、CRP、WBC、ESR、PCT联合CRP、PCT联合WBC及PCT联合ESR特异度比较差异无统计学意义;PCT单独诊断的灵敏度高于CRP、WBC、ESR、PCT联合ESR,PCT单独诊断的准确率高于WBC、ESR、PCT联合WBC和PCT联合ESR,差异有统计学意义(P<0.05)。结论PCT在慢性重型乙型肝炎合并细菌感染患者的诊断灵敏度较高,综合诊断价值突出,值得临床推广应用。Objective To investigate the diagnostic value of serum procalcitonin(PCT)in patients with chronic severe hepatitis B complicated with bacterial infection.Methods The clinical data of 80 patients with severe hepatitis B diagnosed and treated in Jingzhou Hospital of Traditional Chinese Medicine from February 2019 to January 2022 were selected,and they were divided into the infection group and the non-infection group ac-cording to whether they were complicated with bacterial infection,with 40 cases in each group.The levels of serum procalcitonin(PCT),C-reactive protein(CRP),erythrocyte sedimentation rate(ESR)and WBC were compared between the two groups before treatment,and the diagnostic value of PCT,CRP,WBC and ESR in chronic severe hepatitis B with bacterial infection was analyzed.Results Before treatment,the levels of serum PCT,CRP,WBC and ESR in the infection group were higher than those in the non-infection group,and the differences were statistically significant(P<0.05).There were significant differences in the sensitivity and accuracy of PCT,CRP,WBC,ESR,PCT combined with CRP,PCT combined with WBC and PCT combined with ESR(P<0.05);there was no significant difference in the specificity of PCT,CRP,WBC,ESR,PCT combined with CRP,PCT combined with WBC and PCT combined with ESR;the sensitivity of PCT alone was higher than that of CRP,WBC,ESR,PCT combined with ESR,and the accuracy of PCT alone was higher than that of WBC,ESR,PCT combined with WBC and PCT combined with ESR,the difference was statistically significant(P<0.05).Conclusion PCT has high sensitivity and outstanding comprehensive diagnostic value in the diagnosis of pa-tients with chronic severe hepatitis B complicated with bacterial infection,which is worthy of clinical promotion and application.
