化学发光免疫法检测肿瘤生物标志物的临床意义

摘　　要：目的探讨化学发光免疫法在肿瘤生物标志物(TM)检测中的应用价值。方法选取2020年10月至2021年10月于湖南省康复医院诊断为恶性肿瘤的100例患者作为研究组,另选择同期湖南省康复医院行常规体检的50名健康志愿者作为对照组。两组均采用化学发光免疫法检测TM水平,比较两组糖类抗原125(CA125)、癌胚抗原(CEA)、甲胎蛋白(AFP)、神经元特异性烯醇化酶(NSE)、鳞状细胞癌抗原(SCC)、细胞角蛋白19片段21-1(CYFRA21-1)水平,以实验室组织学结果为金标准,分析化学发光免疫法对不同类型恶性肿瘤的检出情况及诊断价值。结果研究组CA125、CEA、AFP、NSE、SCC、CYFRA21-1水平均高于对照组,差异有统计学意义(P<0.05);100例恶性肿瘤中,化学发光免疫法检出恶性肿瘤92例,检出率为92.00%;化学发光免疫法对肺癌、肝癌、胃癌、乳腺癌、宫颈癌、前列腺癌的诊断灵敏度、特异度及准确率均较高,准确率、特异度均>90.00%。结论化学发光免疫法可有效检测不同类型恶性肿瘤患者血清TM水平,可此为依据准确鉴别不同类型恶性肿瘤,且诊断灵敏度和特异度均较高,具有较高的临床应用价值。Objective To investigate the application value of chemiluminescence immunoassay in the detection of tumor biomarkers(TM).Methods 100 patients diagnosed with malignant tumor in Hu'nan Rehabilitation Hospital from October 2020 to October 2021 were selected as the study group,and 50 healthy volunteers who underwent routine physical examination in Hunan Rehabilitation Hospital during the same period were se-lected as the control group.Chemiluminescence immunoassay was used to detect TM levels in the two groups,and the levels of carbohydrate antigen 125(CA125),carcinoembryonic antigen(CEA),alpha fetoprotein(AFP),neuron specific enolase(NSE),squamous cell carcinoma antigen(SCC)and cytokeratin fragment 19(CYFRA21-1)were compared between the two groups,the results of laboratory histology were used as the gold standard to analyze the detection and diagnostic value of chemiluminescence immunoassay for different types of malignant tumors.Results The levels of CA125,CEA,AFP,NSE,SCC and CYFRA21-1 in the study group were higher than those in the control group,and the differences were statistically significant(P<0.05).Among 100 cases of malignant tumors,92 cases were detected by chemiluminescence immunoassay,the detection rate was 92.00%.The sensitivity,specificity and accuracy of chemiluminescence immunoassay in the diagnosis of lung cancer,liver cancer,stomach cancer,breast cancer,cervical cancer and prostate cancer were high,and the accuracy and specificity were more than 90.00%.Conclusion Chemiluminescence immunoas-say can effectively detect the serum TM level of patients with different types of malignant tumors,and can accurately identify different types of ma-lignant tumors based on this,and the diagnostic sensitivity and specificity are high,and has high clinical application value.
