显微外科手术治疗手部皮肤逆行撕脱伤患者临床应用价值分析

摘　　要：目的分析显微镜外科手术治疗手部皮肤逆行撕脱伤患者临床应用价值。方法选取2019年7月至2021年6月庆阳市人民医院收治的76例手部皮肤逆行撕脱伤患者作为研究对象,按照随机数字表法分为观察组与对照组,每组38例。对照组采取常规游离皮片移植术治疗,观察组采取显微外科手术治疗。比较两组手部皮肤存活情况、手部功能恢复情况、手术指标及视觉模拟评分法(VAS)评分。结果观察组手部皮肤逆行撕脱的存活率为92.11%,明显高于对照组的71.05%,差异有统计学意义(P<0.05)。观察组手部功能总恢复率为89.47%,明显高于对照组的63.16%,差异有统计学意义(P<0.05)。观察组手术时间、住院时间均明显短于对照组,出血量明显少于对照组,差异有统计学意义(P<0.05)。术后第1、3天,两组VAS评分均低于术前,且术后第3天低于术后第1天,差异有统计学意义(P<0.05)。术前,两组VAS评分比较差异无统计学意义;术后第1、3天,观察组VAS评分均明显低于对照组,差异有统计学意义(P<0.05)。结论显微镜外科手术可有效治疗手部皮肤逆行撕脱伤患者,改善患者手部情况,安全性较高,值得临床推广应用。Objective To analyze the clinical application value of microsurgery in the treatment of patients with retrograde avulsion of hand skin.Methods 76 patients with retrograde avulsion of hand skin who were admitted to the Qingyang People's Hospital from July 2019 to June 2021 were selected as the research subjects,and they were divided into the observation group and the control group by random number table method,with 38 cases in each group.The control group were treated with conventional free skin grafting,and the observation group were treated with microsur-gery.The hand skin survival,hand functional recovery,surgical indexes and the visual analogue scale(VAS)scores were compared between the two groups.Results The hand skin survival rate in the observation group was 92.11%,which was significantly higher than 71.05%in the control group,the difference was statistically significant(P<0.05).The total hand function recovery rate in the observation group was 89.47%,which was signifi-cantly higher than 63.16%in the control group,the difference was statistically significant(P<0.05).The operation time and hospitalization time in the observation group were significantly shorter than those in the control group,the blood loss was less than that in the control group,the differences were statistically significant(P<0.05).On the 1st and 3rd day after surgery,VAS scores of two groups were lower than before surgery,and the 3rd day after surgery was lower than the 1st day after surgery,the differences were statistically significant(P<0.05).Before operation,there was no sig-nificant difference in VAS scores between the two groups;on the 1st and 3rd day after operation,the VAS scores in the observation group were sig-nificantly lower than those in the control group,the differences were statistically significant(P<0.05).Conclusion Microsurgery is effective in the treatment of patients with retrograde avulsion of hand skin,which can improve the condition of hands,with higher safety,and it is worthy of clinical promotion and application.
