总胆固醇、同型半胱氨酸对甲状腺功能低下的诊断价值

摘　　要：目的:探讨总胆固醇(TC)、同型半胱氨酸(Hcy)对甲状腺功能低下的诊断价值。方法:回顾性选取2020年7月—2022年6月泰州妇产医院门诊收治的150例甲状腺功能低下患者为研究组,另选取甲状腺功能正常患者90例为对照组,比较两组甲状腺功能指标[促甲状腺激素(TSH)、总甲状腺素(TT_(4))]、TC、Hcy。采用Pearson相关性分析甲状腺功能指标与TC、Hcy关系,以受试者工作特征曲线(ROC)下面积(AUC)评估诊断效能。结果:研究组血清TSH、TC、Hcy水平高于对照组,血清TT4水平低于对照组,差异有统计学意义(P<0.05)。Pearson相关分析显示,TSH与TC、Hcy呈正相关,TT4与TC、Hcy呈负相关(P<0.05)。ROC分析结果显示,TC、Hcy水平单一及联合诊断甲状腺功能低下的AUC分别为0.748(95%CI:0.640,0.855)、0.790(95%CI:0.669,0.911)、0.819(95%CI:0.713,0.925)。结论:甲状腺功能低下与TC、Hcy呈正相关,且TC、Hcy水平联合诊断甲状腺功能低下的价值较高。Objective:To investigate the diagnostic value of total cholesterol(TC)and homocysteine(Hcy)in hypothyroidism.Method:A total of 150 patients with hypothyroidism admitted to the Outpatient Department of Taizhou Maternity Hospital from July 2020 to June 2022 were retrospectively selected as the study group,and 90 patients with normal thyroid function were selected as the control group.Thyroid function indexes[thyroid stimulating hormone(TSH),total thyroxine(TT_(4))],TC and Hcy of the two groups were compared.Pearson correlation analysis was used to analyze the relationship between thyroid function indexes and TC and Hcy,and the diagnostic efficacy was evaluated by area under receiver(AUC)operating characteristic curve(ROC).Result:Serum TSH,TC and Hcy levels in the study group were higher than those in the control group,and serum TT4 levels was lower than that in the control group,the differences were statistically significant(P<0.05).Pearson correlation analysis showed that TSH was positively correlated with TC and Hcy,while TT4 was negatively correlated with TC and Hcy(P<0.05).ROC analysis results showed that the AUC of single TC and Hcy levels and combined diagnosis of hypothyroidism were 0.748(95%CI:0.640,0.855),0.790(95%CI:0.669,0.911)and 0.819(95%CI:0.713,0.925),respectively.Conclusion:Hypothyroidism is positively correlated with TC and Hcy,and the combined diagnosis of TSH and TT4 levels has high value in diagnostic hypothyroidism.
