腹腔镜胆囊切除术对胆结石患者的影响

摘　　要：目的:探讨腹腔镜胆囊切除术对胆结石患者的影响。方法:选择2020年1月—2022年1月邳州市中医院收治的100例胆结石患者作为研究对象,采用随机数表法将患者分为对照组与观察组,各50例。对照组采用开腹胆囊切除术治疗,观察组采用腹腔镜胆囊切除术治疗,比较两组术后恢复情况、应激反应、胃肠激素水平及术后并发症发生情况。结果:观察组引流管留置时间、疼痛持续时间、首次排气时间、住院时间均优于对照组,差异有统计学意义(P<0.05)。两组术前应激反应、胃肠激素水平比较,差异无统计学意义(P>0.05);观察组术后内皮素、皮质醇、胃泌素水平及并发症发生率低于对照组,血清超氧化物歧化酶、胃动素水平高于对照组,差异有统计学意义(P<0.05)。结论:腹腔镜胆囊切除术相比开腹胆囊切除术,能有效缩短胆结石患者的恢复时间、疼痛持续时间、住院时间,减轻应激反应,改善胃肠激素水平,并降低术后并发症风险。Objective:To explore the impact of laparoscopic cholecystectomy on patients with gallstones.Method:A total of 100 patients with gallstones who admitted to Pizhou Traditional Chinese Medicine Hospital from January 2020 to January 2022 were selected as the research subjects,the patients were divided into the control group and the observation group by random number table method,50 cases in each group.The control group was underwent open cholecystectomy,while the observation group was underwent laparoscopic cholecystectomy.The postoperative recovery,stress response,gastrointestinal hormone levels and the incidence of postoperative complications in two groups were compared.Result:The drainage tube indwelling time,pain duration,first exhaust time and hospitalization time in the observation group were better than those in the control group,and the differences were statistically significant(P<0.05).There were no significant differences in preoperative stress response and gastrointestinal hormone levels between two groups(P>0.05);the levels of endothelin,cortisol,gastrin and the incidence of complications in the observation group were lower than those in the control group,and the levels of serum superoxide dismutase and motilin were higher than those in the control group,the differences were statistically significant(P<0.05).Conclusion:Compared with open cholecystectomy,laparoscopic cholecystectomy can effectively shorten the recovery time,pain duration and hospital stay of patients with gallstones,alleviate stress response,improve gastrointestinal hormone levels,and reduce the risk of postoperative complications.
