雷火灸联合加味苓桂术甘汤对后循环缺血性眩晕的疗效

摘　　要：目的:探讨雷火灸联合加味苓桂术甘汤对后循环缺血性眩晕(PCIV)的疗效。方法:选择2021年4月—2022年12月在江门市新会区中医院神经内科或内科五区按相关诊断标准确诊为PCIV的60例住院患者作为研究对象,根据随机数表法将患者分为对照组和观察组,各30例。对照组给予西医常规治疗,观察组对照组基础上加以雷火灸联合加味苓桂术甘汤治疗,比较两组患者中医症候积分、临床疗效及椎-基底动脉血流动力学。结果:治疗后,两组中医症候积分低于治疗前,且观察组低于对照组,差异有统计学意义(P<0.05)。观察组总有效率(86.7%)高于对照组(56.7%),差异有统计学意义(P<0.05)。治疗后,两组双侧椎动脉、基底动脉血流速度(VM)高于治疗前,且观察组高于对照组,差异有统计学意义(P<0.05),但两组治疗前后双侧椎动脉、基底动脉血管搏动指数(PI)比较,差异无统计学意义(P>0.05)。结论:雷火灸联合加味苓桂术甘汤治疗PCIV可有效改善患者微循环,增加动脉血管顺应性,进而提高临床疗效。Objective:To explore the effect of thunder-fire moxibustion combined with modified Linggui Zhugan Decoction on posterior circulation ischemic vertigo(PCIV).Method:A total of 60 inpatients diagnosed with PCIV according to the relevant diagnostic criteria in the Department of Neurology or the Fifth District of Internal Medicine of Jiangmen City Xinhui District Traditional Chinese Medicine Hospital from April 2021 to December 2022 were selected as the study objects,the patients were randomly divided into the control group and the observation group according to the random number table method,30 cases in each group.The control group was treated with conventional western medicine,and the observation group was treated with thunder-fire moxibustion combined with modified Linggui Zhugan Decoction on the basis of the control group,the traditional Chinese medicine(TCM)symptom scores,clinical efficacy and vertebral-basilar artery hemodynamics were compared between two groups.Result:After treatment,the TCM symptom scores in two groups were lower than those before treatment,and that in the observation group was lower than that in the control group,the differences were statistically significant(P<0.05).The total effective rate of the observation group(86.7%)was higher than that of the control group(56.7%),and the difference was statistically significant(P<0.05).After treatment,the blood flow velocity(VM)of bilateral vertebral artery and basilar artery in two groups were higher than those before treatment,and those in the observation group were higher than those in the control group,the differences were statistically significant(P<0.05);however,there were no significant differences in the pulsatility index(PI)of bilateral vertebral artery and basilar artery between two groups before and after treatment(P>0.05).Conclusion:Thunder-fire moxibustion combined with modified Linggui Zhugan Decoction in the treatment of PCIV can effectively improve the microcirculation of patients,increase arterial compliance,and thus improve the clinic
