右美托咪定联合罗哌卡因在老年全髋关节置换术患者中的临床应用价值

摘　　要：目的:研究右美托咪定联合罗哌卡因在老年全髋关节置换术患者中的临床应用效果。方法:选择2020年1月—2022年1月黄冈市黄州区人民医院收治的120例行全髋关节置换术的老年患者为研究对象,使用电脑分组法分为研究组与常规组,各60例。两组均接受超声引导下髂筋膜间隙阻滞麻醉,常规组使用罗哌卡因,研究组在常规组的基础上加用右美托咪定。比较两组术后认知功能、应激反应情况、疼痛评分、麻醉时间与并发症发生率。结果:术后1 d、7 d,两组认知功能障碍率比较,差异无统计学意义(P>0.05)。麻醉前、用药后10 min,两组心率(HR)、平均动脉压(MAP)比较,差异无统计学意义(P>0.05);用药后30 min、用药后1 h及手术结束时,研究组HR、MAP明显低于常规组,差异有统计学意义(P<0.05)。术后30 min、1 h,研究组视觉模拟评分法(VAS)评分均低于常规组,差异有统计学意义(P<0.05)。研究组麻醉时间为(5.67±1.94)min,明显短于常规组的(11.55±2.05)min,差异有统计学意义(t=16.137,P<0.001)。两组并发症总发生率比较,差异无统计学意义(P>0.05)。结论:右美托咪定联合罗哌卡因在老年全髋关节置换术患者中有较好的临床应用效果,不仅能大大提高患者的麻醉效果,还具有良好的安全性。Objective:To study the clinical application effect of Dexmedetomidine combined with Ropivacaine in elderly patients undergoing total hip arthroplasty.Method:A total of 120 elderly patients undergoing total hip arthroplasty admitted to Huangzhou District People's Hospital of Huanggang City from January 2020 to January 2022 were selected as the study objects.They were divided into study group and conventional group by computer grouping method,with 60 cases in each group.Both groups received ultrason-guided iliac fascia space block anesthesia,the conventional group was given Ropivacaine,and the study group was given Dexmedetomidine in addition to the conventional group.Postoperative cognitive function,stress response,pain score,anesthesia time and complication rate were compared between the two groups.Result:At 1 d and 7 d after surgery,there were no significant differences in the rate of cognitive dysfunction between the two groups(P>0.05).Before anesthesia and 10 min after medication,there were no significant differences in heart rate(HR)and mean arterial pressure(MAP)between the two groups(P>0.05).At 30 min after medication,1 h after medication and at the end of surgery,HR and MAP in the study group were significantly lower than those in the conventional group,and the differences were statistically significant(P<0.05).At 30 min and 1 h after surgery,visual analogue scale(VAS)scores in the study group were lower than those in the conventional group,and the differences were statistically significant(P<0.05).The duration of anesthesia in the study group was(5.67±1.94)min,which was significantly shorter than(11.55±2.05)min in the conventional group,and the difference was statistically significant(t=16.137,P<0.001).There was no significant difference in the total incidence of complications between the two groups(P>0.05).Conclusion:Dexmedetomidine combined with Ropivacaine has a good clinical application effect in elderly patients undergoing total hip arthroplasty,which not only can greatly improve the anesthetic eff
