布地奈德雾化吸入联合NCPAP治疗NP的效果及对IL-33与呼吸功能的影响

摘  要:目的:分析布地奈德雾化吸入联合经鼻持续气道正压通气(nasal continuous positive airway pressure,NCPAP)治疗新生儿肺炎(neonatal pneumonia,NP)的效果及对白细胞介素-33(interleukin-33,IL-33)与呼吸功能的影响。方法:选取2022年7月—2023年7月淮安市妇幼保健院收治的184例NP患儿。根据随机数表法将其分为NCPAP组和联合组,各92例。NCPAP组给予NCPAP,联合组在NCPAP组的基础上给予布地奈德雾化吸入。比较两组治疗前后炎症因子、呼吸功能,临床疗效。结果:治疗后,联合组IL-33、白细胞介素-6(interleukin-6,IL-6)、降钙素原(procalcitonin,PCT)、C反应蛋白(C-reactive protein,CRP)水平均低于NCPAP组,差异有统计学意义(P<0.05)。治疗后,联合组呼吸频率低于NCPAP组,每公斤体重潮气量、达峰时间比、达峰容积比均高于NCPAP组,差异有统计学意义(P<0.05)。联合组总有效率高于NCPAP组,差异有统计学意义(P<0.05)。结论:布地奈德雾化吸入联合NCPAP治疗NP效果较好,可减轻患儿炎症反应,提高患儿呼吸功能,促进患儿身体恢复。Objective:To analyze the effect of Budesonide nebulizing inhalation combined with nasal continuous positive airway pressure(NCPAP)in the treatment of neonatal pneumonia(NP)and its influence on interleukin-33(IL-33)and respiratory function.Method:A total of 184 children with NP treated in Huai’an Maternal and Child Health Hospital from July 2022 to July 2023 were selected.According to random number table method,they were divided into NCPAP group and combined group,with 92 cases in each group.The NCPAP group was given NCPAP,and the combined group was given Budesonide nebulizing inhalation on the basis of NCPAP group.Inflammatory factors,respiratory function before and after treatment and clinical efficacy of the two groups were compared.Result:After treatment,the levels of IL-33,interleukin-6(IL-6),procalcitonin(PCT)and C-reactive protein(CRP)in the combined group were lower than those in the NCPAP group,the differences were statistically significant(P<0.05).After treatment,the respiratory rate of the combined group was lower than that of the NCPAP group,and the tidal volume per kilogram of body weight,peak time ratio,and peak volume ratio were higher than those of the NCPAP group,the differences were statistically significant(P<0.05).The total effective rate of combined group was higher than that of NCPAP group,and the difference was statistically significant(P<0.05).Conclusion:Budesonide nebulizing inhalation combined with NCPAP is effective in the treatment of NP,which can reduce the inflammatory response,improve the respiratory function of children and promote the physical recovery of the children.
