五苓散联合指压法按摩风池穴治疗痰饮型颈性头痛临床观察

摘　　要：目的:分析五苓散联合指压法按摩风池穴治疗痰饮型颈性头痛的效果。方法:将2020年3月—2023年2月河源市中医院收治的60例痰饮型颈性头痛患者作为研究对象,根据随机数表法将患者分为对照组(n=30)与试验组(n=30)。对照组给予西比灵胶囊治疗,试验组在对照组基础上给予五苓散联合指压法按摩风池穴。比较两组治疗前、治疗4周后头痛程度、头痛发作频率及持续时间相关评分、头痛影响测评量表-6(HIT-6)评分、颈部血流动力学指标(基底动脉、左侧椎动脉、右侧椎动脉平均流速)。结果:两组治疗前头痛程度、头痛发作频率及持续时间相关评分、HIT-6评分、颈部血流动力学指标比较,差异无统计学意义(P>0.05);试验组治疗4周后头痛程度评分、头痛发作频率及持续时间相关评分、HIT-6评分低于对照组,基底动脉、左侧椎动脉、右侧椎动脉平均流速高于对照组,差异有统计学意义(P<0.05)。结论:五苓散联合指压法按摩风池穴可显著改善痰饮型颈性头痛患者头痛程度和频率,改善颈部血流。Objective:To analyze the effect of Wuling Powder combined with finger pressing massage at fengchi point in the treatment of patients with phlegm Yin type cervical headache.Method:A total of 60 patients with phlegm Yin type cervical headache who admitted to Heyuan Hospital of Traditional Chinese Medicine from March 2020 to February 2023 were selected as the research objects,the patients were divided into the control group(n=30)and experimental group(n=30)according to the random number table method.The control group was treated with Sibelium Capsules,and the experimental group was treated with Wuling Powder combined with finger pressing massage at fengchi point on the basis of the control group.The headache degree,headache frequency and duration related score,headache impact assessment scale-6(HIT-6)score,neck hemodynamics indexes(the average flow velocity of basilar artery,left vertebral artery,right vertebral artery)before and after 4 weeks of treatment were compared between two groups.Result:There were no significant differences in the headache degree score,headache attack frequency and duration related score,HIT-6 score and cervical hemodynamic indexes between two groups before treatment(P>0.05);the headache degree score,headache attack frequency and duration related score,HIT-6 score in the experimental group after 4 weeks of treatment were lower than those in the control group,and the average flow velocity of basilar artery,left vertebral artery and right vertebral artery were higher than those in the control group,the differences were statistically significant(P<0.05).Conclusion:Wuling Powder combined with finger pressing massage at fengchi point can significantly improve the degree and frequency of headache in patients with phlegm Yin type cervical headache,and improve cervical blood flow.
