柴芩承气汤治疗湿热壅滞型急性胰腺炎的临床观察

摘　　要：目的:研究柴芩承气汤治疗湿热壅滞型急性胰腺炎的临床疗效。方法:选取2019年1月—2022年10月盱眙县中医院收治的90例湿热壅滞型急性胰腺炎患者,按照随机数表法分为研究组(45例)和对照组(45例),对照组采用奥曲肽进行治疗,研究组采用柴芩承气汤治疗,比较两组方案的临床疗效。结果:研究组治疗总有效率高于对照组,差异有统计学意义(P<0.05)。治疗后,两组中医症候积分各维度评分均下降,且研究组低于对照组,差异有统计学意义(P<0.05)。治疗后,两组C反应蛋白(CRP)、降钙素原(PCT)水平均降低,且研究组低于对照组,差异有统计学意义(P<0.05)。治疗后,两组血清白蛋白(ALB)水平上升,血尿素氮(BUN)、总胆红素(TBIL)水平下降,且研究组ALB水平高于对照组,BUN、TBIL水平低于对照组,差异有统计学意义(P<0.05)。研究组面红肿胀、消化道出血、皮疹等不良反应总发生率低于对照组,差异有统计学意义(P<0.05)。结论:柴芩承气汤治疗湿热壅滞型急性胰腺炎效果显著,能够促进患者体内炎症反应与肝肾功能改善,有助于缓解其临床症状,治疗安全性较高。Objective:To study the clinical efficacy of Chaiqin Chengqi Decoction in treating acute pancreatitis with damp heat stagnation type.Method:A total of 90 patients with acute pancreatitis of damp heat stagnation type treated in Xuyi Hospital of Traditional Chinese Medicine from January 2019 to October 2022 were selected,they were divided into study group(45 cases)and control group(45 cases)according to random number table method.The control group was treated with Octreotide,and the study group was treated with Chaiqin Chengqi Decoction.The clinical efficacy of the two groups was compared.Result:The total effective rate of the study group was higher than that of the control group,the difference was statistically significant(P<0.05).After treatment,the scores of traditional Chinese medicine syndrome scores of both groups decreased in all dimensions,and the study group were lower than those of the control group,the differences were statistically significant(P<0.05).After treatment,C-reactive protein(CRP)and procalcitonin(PCT)levels were decreased in both groups,and the study group were lower than those of the control group,the differences were statistically significant(P<0.05).After treatment,serum albumin(ALB)levels increased,blood urea nitrogen(BUN)and total bilirubin(TBIL)levels decreased,and ALB level in the study group was higher than that in the control group,BUN and TBIL levels were lower than those in the control group,the differences were statistically significant(P<0.05).The total incidence of adverse reactions such as facial redness and swelling,gastrointestinal bleeding and rash in the study group was lower than that in the control group,the difference was statistically significant(P<0.05).Conclusion:Chaiqin Chengqi Decoction has a significant effect in the treatment of acute pancreatitis of damp heat stagnation type,which can promote the improvement of inflammation and liver and kidney function in patients,and help alleviate clinical symptoms,with high treatment safety.
