植皮手术联合瘢痕整形对烧伤后患者美容效果的影响

摘　　要：目的 探讨植皮手术联合瘢痕整形对烧伤后患者美容效果的影响。方法 选取80例深度烧伤后整形患者为研究对象,选择随机数字表法分为研究组和参照组,每组40例。研究组患者采用植皮手术联合瘢痕整形治疗,参照组患者采用单一植皮手术治疗。比较两组患者二次手术率及皮片成活率、瘢痕情况、术后瘢痕增生程度及瘢痕典型案例发生情况、满意度。结果 二次手术率比较,研究组(2.50%)显著低于参照组(25.00%)(P<0.05);皮片成活率比较,研究组(97.50%)显著高于参照组(65.00%)(P<0.05)。两组术前厚度、色泽、血管分布、柔软度评分比较无显著差异(P>0.05);两组术后厚度、色泽、血管分布、柔软度评分低于术前(P<0.05);研究组术后厚度、色泽、血管分布、柔软度评分分别为(1.98±0.11)、(1.11±0.08)、(1.12±0.28)、(1.34±0.32)分,低于参照组的(2.59±0.12)、(1.85±0.12)、(1.39±0.12)、(1.65±0.12)分(P<0.05)。术后瘢痕增生情况比较,研究组优于参照组(P<0.05);瘢痕典型案例发生率比较,研究组(0)显著低于参照组[15.00%(6/40)](P<0.05)。满意度比较,研究组(97.50%)显著高于参照组(75.00%)(P<0.05)。结论 深度烧伤后整形患者选择植皮手术联合瘢痕整形治疗方案,美容效果好,价值显著,可推广。Objective To discuss the effect of skin grafting surgery combined with scar plastic surgery on the beauty of burn patients.Methods 80 patients with deep burn after plastic surgery were selected as the study subjects,and divided into a study group and a reference group by digital table method,with 40 cases in each group.The study group was treated with skin grafting surgery combined scar plastic surgery,and the reference group was treated with single skin grafting surgery.The secondary surgery rate,survival rate of skin graft,scar condition and the degree of postoperative scar hyperplasia,typical scar occurrence and satisfaction were compared between the two groups.Results The secondary surgery rate in the study group(2.50%) was significantly lower than that in the reference group(25.00%)(P<0.05).The survival rate of skin graft in the study group(97.50%) was significantly higher than that in the reference group(65.00%)(P<0.05).There were no significant differences in scores of preoperative thickness,color,vascular distribution and flexibility between the two groups(P>0.05).The scores of preoperative thickness,color,vascular distribution and flexibility in both groups were lower than those before surgery(P<0.05).The scores of postoperative thickness,color,vascular distribution and flexibility in the study group were(1.98±0.11),(1.11±0.08),(1.12±0.28) and(1.34±0.32) points,which were lower than those of(2.59±0.12),(1.85±0.12),(1.39±0.12) and(1.65±0.12) points in the reference group(P<0.05).Postoperative scar hyperplasia in the study group was better than that in the reference group(P<0.05).The incidence of typical scar in the study group(0) was significantly lower than that in the reference group [15.00%(6/40)](P<0.05).The satisfaction of the study group(97.50%) was significantly higher than that of the reference group(75.00%)(P<0.05).Conclusion The combination of skin grafting and scar plastic surgery has good beauty,significant value and can be popularized.
