摘 要:目的:探讨改良骶髂关节MRI扫描技术对强直性脊柱炎患者骶髂关节病变与关节损伤程度的评估价值。方法:选自2022年7月-2023年7月我院门诊及住院强直性脊柱炎患者90例,根据强直性脊柱炎患者病情进展不同分为强直性脊柱炎进展期(进展期组,45例)与强直性脊柱炎强直期(强直期组,45例)。对所有患者给予改良骶髂关节MRI扫描技术(多模态骶髂关节MRI扫描)检查,与临床确诊结果比较,分析其对不同损伤程度骶髂关节病变检出情况。结果:进展期组患者临床确诊骶髂关节病变总检出率(86.67%,39/45)与改良骶髂关节MRI扫描技术总检出率(77.78%,35/45)比较,差异无统计学意义(P>0.05)。强直期组患者临床确诊骶髂关节病变总检出率(100.00%,45/45)与改良骶髂关节MRI扫描技术总检出率(95.56%,43/45)比较,差异无统计学意义(P>0.05)。结论:多模态骶髂关节MRI扫描技术对骶髂关节病变诊断效果较好,与临床确诊结果无显著差异,可实现强直性脊柱炎获得更早期、更精准的诊断,为临床诊疗提供更有价值的医学影像诊断分析和依据。Objective:To explore the value of improved sacroiliac joint MRI scanning technology in evaluating the degree of sacroiliac joint lesions and joint injuries in patients with ankylosing spondylitis.Methods:90 patients with ankylosing spondylitis were selected from our hospital's outpatient and inpatient departments from July 2022 to July 2023.According to the different progression of ankylosing spondylitis patients,they were divided into the progressive stage of ankylosing spondylitis(45 cases in the progressive stage group)and the ankylosing stage of ankylosing spondylitis(45 cases in the ankylosing stage group).All patients were examined using a modified sacroiliac joint MRI scanning technique(multimodal sacroiliac joint MRI scanning),and compared with clinical confirmed results to analyze the detection of sacroiliac joint lesions with different degrees of injury.Results:The total detection rate of clinically confirmed sacroiliac joint lesions in patients in the advanced stage group(86.67%,39/45)was not statistically significant compared to the total detection rate of the improved sacroiliac joint MRI scanning technology(77.78%,35/45)(P>0.05).The total detection rate of clinically confirmed sacroiliac joint lesions in patients in the ankylosing phase group(100.00%,45/45)was not statistically significant compared to the total detection rate of the improved sacroiliac joint MRI scanning technique(95.56%,43/45)(P>0.05).Conclusion:The multimodal sacroiliac joint MRI scanning technology has a good diagnostic effect on sacroiliac joint lesions,and there is no significant difference with clinical diagnosis results.It can achieve earlier and more accurate diagnosis of ankylosing spondylitis,providing more valuable medical imaging diagnostic analysis and basis for clinical diagnosis and treatment.
