骨质疏松性椎体压缩性骨折患者MRI信号与PVP治疗效果的关系分析

摘　　要：目的:探讨骨质疏松性椎体压缩性骨折(OVCF)患者磁共振成像(MRI)信号与经皮椎体成形术(PVP)治疗效果的关系分析。方法:选取2020年10月至2023年10月在本院行PVP术治疗的OVCF患者82例为研究对象,均行MRI检查,根据MRI信号骨髓水肿程度分为无骨髓水肿组、部分骨髓水肿组和完全骨髓水肿组,共3组。比较3组围手术期视觉模拟疼痛评分(VAS)、伤椎高度、Cobb角、Oswestry功能障碍指数量表(ODI)评分以及椎体再骨折发生率。结果:术后1周、3个月,完全骨髓水肿组VAS评分、ODI评分及Cobb角均低于部分骨髓水肿组和无骨髓水肿组,椎体前缘高度、中间高度、后缘高度均高于部分骨髓水肿组和无骨髓水肿组(P<0.05)。完全骨髓水肿组椎体再骨折发生率为0,低于部分骨髓水肿组的(5.26%)和无骨髓水肿组(20.00%)(P<0.05)。结论:MRI信号特征与OVCF患者PVP治疗效果存在一定相关性,MRI骨髓水肿范围越大,PVP治疗效果越好,椎体再骨折发生率也越低。Objective:To explore the relationship between magnetic resonance imaging(MRI)signals and the therapeutic effect of percutaneous vertebroplasty(PVP)in patients with osteoporotic vertebral compression fractures(OVCF).Methods:82 OVCF patients who underwent PVP surgery in our hospital from October 2020to October 2023 were selected as the study subjects,all of whom underwent MRI examination.According to the degree of MRI signal bone marrow edema,they were divided into three groups:no bone marrow edema group,partial bone marrow edema group,and complete bone marrow edema group.Compare the perioperative visual analog pain score(VAS),vertebral height,Cobb angle,Oswestry Dysfunction Index(ODI)score,and vertebral body re fracture incidence among three groups.Results:At 1 week and 3 months after surgery,the VAS score,ODI score,and Cobb angle of the complete bone marrow edema group were lower than those of the partial bone marrow edema group and the non bone marrow edema group.The anterior,middle,and posterior heights of the vertebral body were higher than those of the partial bone marrow edema group and the non bone marrow edema group(P<0.05).The incidence of vertebral re fracture in the complete bone marrow edema group was 0,lower than that in the partial bone marrow edema group(5.26%)and the non bone marrow edema group(20.00%)(P<0.05).Conclusion:There is a certain correlation between MRI signal characteristics and the therapeutic effect of PVP in OVCF patients.The larger the range of MRI bone marrow edema,the better the therapeutic effect of PVP,and the lower the incidence of vertebral re fracture.
