摘 要:目的探讨对屈肌腱断裂采用显微改良凯斯勒缝合法治疗的效果及安全性。方法选择2018年1月—2023年4月本院收治屈肌腱断裂患者70例,随机分为常规缝合治疗对照组(n=35)与采用显微改良凯斯勒缝合治疗观察组(n=35),比较两组手术相关指标,评估术后患者手部肌腱恢复优良率,对比患者术后2周、4周的握力与捏力水平,并统计并发症率进行对比分析。结果观察组手术时间、住院时间、术后VAS评分均小于对照组(P<0.05)。观察组手部肌腱恢复优良率较对照组高(P<0.05)。术后2周、4周,观察组握力、捏力均大于对照组(P<0.05)。观察组并发症率较对照组更低(P<0.05)。结论对屈肌腱断裂采用显微改良凯斯勒缝合治疗能减轻术后疼痛,缩短住院时间,可改善手部功能,提高握力与捏力,且可降低并发症风险。Objective To explore effect and safety of microsurgical modified Kessler suture method for flexor tendon rupture.Methods The paper chose 70 patients with flexor tendon rupture admitted to the hospital from January 2018 to April 2023,and divided them into control group(n=35)treated with routine suture and observation group with micro modified Kessler suture(n=35)randomly.Surgical related indicators between two groups were compared,excellent and good recovery rate of hand tendon was evaluated after surgery.Grip and pinch strength levels of patients at 2 and 4 weeks after surgery were compared,and incidence of complications was statistically analyzed for comparison.Results Observation group had shorter surgical time(63.51±6.84)minutes,hospital stay(19.31±2.18),and postoperative VAS scores(3.18±1.05)than control group(P<0.05).Excellent and good recovery rate of hand tendons in observation group was higher than control group(94.29%vs.77.14%,P<0.05).At 2 and 4 weeks after surgery,grip strength of observation group was(9.40±1.64)kg,(14.16±2.12)kg,and pinch strength was(2.58±0.51)kg,(3.68±0.55)kg,all of which were higher than control group(P<0.05).Complication incidence in observation group was lower than control group(5.71%vs.34.29%,P<0.05).Conclusion Microsurgical modified Kessler suture treatment can reduce postoperative pain,shorten hospital stay,improve hand function,increase grip and pinch strength,and reduce complications risks of flexor tendon rupture.
