PFNA与人工股骨头置换术治疗老年不稳定性股骨粗隆间骨折的临床比较

摘　　要：目的探讨PFNA和人工股骨头置换术在老年不稳定性股骨粗隆间骨折中的治疗效果。方法选取2021年11月—2023年2月本院收治的老年不稳定性股骨粗隆间骨折患者103例为研究对象,并将患者按数字表法分为对照组(51例)和观察组(52例)。其中,对照组采取人工股骨头置换术,观察组采取股骨近端防旋髓内钉。将两组患者手术指标、术后不同时间段中髋关节功能评分、并发症情况进行比较。结果观察组患者在手术指标对比中,各项手术指标数据均较对照组患者手术指标数据更优,差异有统计学意义(P<0.05);术后1个月、术后3个月、术后6个月时髋关节功能分数对比中,观察组患者分数较对照组髋关节功能分数更高,差异具有统计学意义(P<0.05);观察组患者手术后并发症发生率显著低于对照组,差异存在统计学意义(P<0.05)。结论股骨近端防旋髓内钉的应用较人工股骨头置换术效果更好,手术治疗后,老年不稳定性股骨粗隆间骨折患者髋关节功能恢复更好,且安全性更高,并发症更少,可在临床中广泛应用。Objective To explore curative effect of PFNA and artificial femoral head replacement surgery for unstable intertrochanteric fractures of the elderly.Methods The paper chose 103 elderly patients with unstable intertrochanteric fractures admitted to our hospital from November 2021 to February 2023,and divided them into two groups with digital table method,51 cases in control group were treated with artificial femoral head replacement surgery,52 cases in observation group with proximal femoral anti rotation intramedullary nails.Surgical indicators,hip joint function scores,and complications were compared between two groups at different postoperative time periods.Results Surgical indicators in observation group were better than control group(P<0.05),difference was statistically significant.Hip joint function scores at 1 month,3 months,and 6 months after surgery in observation group were higher than control group(P<0.05),difference was statistically significant.Incidence of postoperative complications in observation group was 3.85%,significantly lower than 21.57% in control group,difference was statistically significant(P<0.05).Conclusion Proximal femoral anti rotation intramedullary nails can achieve better effect than artificial femoral head replacement surgery.After surgical treatment,hip joint function recovery is better for elderly patients with unstable intertrochanteric fractures,with higher safety and fewer complications,which can be widely applied in clinical practice.
