TAPP术与Bassini修补术在腹股沟疝患者中临床疗效对比

摘　　要：目的对腹股沟疝患者分别采用腹腔镜经腹腹膜前修补术(TAPP)与右斜疝修补术(Bassini)并探讨两者的临床疗效。方法抽取2019年5月—2022年5月本院收治的96例腹股沟疝患者作为本次研究对象,并将其按随机数字表法分为TAPP组与Bassini组,每组48例。TAPP组患者行TAPP术,Bassini组行Bassini术。对比两组患者围术期指标、疼痛程度及并发症发生率。结果研究结果显示,TAPP组手术时间长于Bassini组、术中出血量低于Bassini组、术后下床活动时间及住院时间均短于Bassini组(P<0.05);两组患者术后即刻、术后30d疼痛程度对比差异不大(P>0.05),TAPP组术后24h、术后72h、术后7d、术后15d疼痛程度显著低于Bassini组(P<0.05);TAPP组并发症总发生率显著低于Bassini组(P<0.05)。结论TAPP术在腹股沟疝患者应用中,具有并发症发生率低、创伤小、疼痛程度低等优势,且TAPP术操作与Bassini术相比更为简单,因此临床疗效更为显著。Objective To investigate clinical effect of laparoscopic transabdominal anterior peritoneal repair(TAPP)and right oblique hernia repair(Bassini)for patients with inguinal hernia.Methods The paper chose 96 patients with inguinal hernia in our hospital as study subjects,with sampling time range from May 2019 to May 2022,and divided them into TAPP group and basini group randomly,with 48 cases in each group.TAPP group was treated with TAPP surgery,while basini group with basini surgery.Perioperative indicators,degree of pain,and incidence of complications was compared between two groups.Results TAPP group had longer surgical time,lower intraoperative bleeding volume(ml),shorter postoperative ambulatory time and length of stay than basini group(P<0.05).Comparison of pain levels between two groups immediately and 30 days after surgery showed(P>0.05).Pain level in TAPP group was significantly lower than basini group at 24 hours,72 hours,7 days,and 15 days after surgery(P<0.05).Total incidence of complications in TAPP group was 4.17%,significantly lower than 25.00% in basini group(P<0.05).Conclusion Comparison of two surgical treatments for inguinal hernia patients concludes TAPP has advantages including low incidence of complications,minimal trauma,and low pain.Moreover,TAPP procedure is simpler with more significant clinical effect than basini.
