摘  要:目的:研究影响肥胖儿童发生高尿酸血症的临床因素及尿酸水平与肥胖儿童人体成分的关系。方法:回顾性分析2018年1月至2022年12月在宁夏医科大学总医院儿科门诊及住院部确诊为单纯性肥胖症的儿童171例,根据空腹血尿酸水平分为高尿酸血症儿童和非高尿酸血症儿童,比较两组临床资料及人体成分之间的差异。并对其进行单因素、多因素分析,同时对肥胖儿童尿酸水平与其他观察指标进行相关分析。结果:多因素logistic回归结果显示,体脂百分比、肌酐、血钙、血磷、碱性磷酸酶指标升高是肥胖儿童发生高尿酸血症的独立危险因素,25羟基维生素D升高是肥胖儿童发生高尿酸血症的保护因素(P均<0.05)。肥胖儿童尿酸水平与内脏脂肪面积、体脂百分比、血钙、肌酐、碱性磷酸酶呈正相关(r均> 0,P均<0.01)。结论:体质量指数相同的肥胖儿童,体脂百分比、内脏脂肪面积值越大,越容易发生高尿酸血症;此外,肌酐、血钙、血磷、碱性磷酸酶指标升高为肥胖儿童发生高尿酸血症的危险因素,25羟基维生素D升高是保护因素。Objective To study the clinical factors influencing the occurrence of hyperuricemia in obese children and the relationship between uric acid level and body composition in obese children.Methods A total of 171 children diagnosed with simple obesity in the pediatric outpatient department and inpatient department of General Hospital of Ningxia Medical University from January 2018 to December 2022 were retrospectively analyzed.They were divided into hyperuricemia children and non-hyperuricemia children according to fasting blood uric acid level,and the differences in clinical data and body composition between the two groups were compared.Univariate and multivariate analysis was performed,and the correlation between uric acid level and other observational indicators in obese children was analyzed.Results Multivariate logistic regression results showed that the increase of body fat percentage,creatinine,blood calcium,blood phosphorus and alkaline phosphatase indexes were the independent risk factors for hyperuricemia in obese children,and the increase level of 25-hydroxyvitamin D was a protective factor for hyperuricemia in obese children(P<0.05).The uric acid level in obese children was positively correlated with visceral fat area,body fat percentage,blood calcium,creatinine and alkaline phosphatase(r>0,P<0.01).Conclusion In obese children with the same body mass index,the higher the percentage of body fat and the area of visceral fat,the more likely to develop hyperuricemia.In addition,increased levels of creatinine,serum calcium,serum phosphorus and alkaline phosphatase were the risk factors for hyperuricemia in obese children,and increased level of 25-hydroxyvitamin D were the protective factors.
