摘　　要：目的临床试验报道,多种油脂肪乳(SMOF)对免疫功能有积极作用,但与其他脂质乳剂相比较结果不一致,使用快速卫生技术评估的方法为临床用药决策提供循证医学证据。方法计算机检索the Cochrane Library、PubMed、Embase、中国知网、万方数据库,收集SMOF用于成人住院患者的系统评价/Meta分析和药物经济学研究。结果共纳入7项研究,包括5项Meta分析和2项药物经济学研究。有效性方面:所纳入的5篇Meta分析结果基本一致,总体上SMOF对丙氨酸氨基转移酶(ALT)、天门冬氨酸氨基转移酶(AST)、γ-谷氨酰基转移酶(GGT)、总胆红素(TB)、甘油三酯(TG)等肝功能标志物血清浓度都有不同程度的积极效果,可以缩短住院时间(LOS),改善临床结局。没有观察到SMOF在改善炎症指标C反应蛋白(CRP)及白介素-6(IL-6)方面有显著影响。安全性方面:纳入研究均表明SMOF的不良事件(AE)发生率与其他脂肪乳剂相当。经济性方面:2项药物经济学研究均提示,SMOF具有显著的经济学优势。结论SMOF用于成人住院患者其有效性、安全性和经济性有显著优势,具有较高的临床价值。Objective Clinical trials reported that multiple oil-fat emulsions(SMOF)had a positive effect on immune function,but the results were inconsistent compared with other lipid emulsions.The method of rapid health technology assessment was used to provide evidencebased medical evidence for clinical medication decisions.Methods Computer searches were conducted on the Cochrane Library,PubMed,Embase,China National Knowledge Infrastructure,and Wanfang Database to collect systematic reviews/Meta-analyses and pharmacoeconomic studies of SMOF used in adult inpatients.Results A total of 7 studies were included,including 5 Meta-analyses and 2 pharmacoeconomic studies.In terms of effectiveness:the results of the 5 included Meta-analyses are basically consistent.Generally,SMOF has varying degrees of positive effect on alanine aminotransferase(ALT),aspartate aminotransferase(AST),and gamma-glutamyl transferase(GGT),total bilirubin(TB),triglyceride(TG),etc.liver function markers serum concentrations,which can shorten hospitalization time(LOS)and improve clinical outcomes.No significant effect of SMOF was observed in improving inflammatory markers C-reactive protein(CRP)and interleukin-6(IL-6).Safety:The included studies have shown that the incidence of adverse events(AE)of SMOF is comparable to that of other fat emulsions.Economy:Two pharmacoeconomic studies all suggest that SMOF has significant economic advantages.Conclusion SMOF has significant advantages in effectiveness,safety and economy when used in adult inpatients,and has high clinical value.
