驾照式计分管理对某院甲状腺围术期预防用抗菌药物的影响分析

摘　　要：目的研究驾照式计分管理对某院2017~2020年度甲状腺手术围术期预防性应用抗菌药物使用情况的影响。方法共纳入调查病例503例,分别记录患者的年龄、性别、诊断、抗菌药物品种选择、给药时机选择、使用时间等数据并分析。结果2017年(驾照式管理前)抗菌药物预防使用率高于30%(31.78%),其余年份均低于国家要求的<30%;2017年抗菌药物预防使用率分别与其他年份进行对比,差异均具有统计学意义(P<0.01);2017年术前给药时机合理率仅为55.88%,2018~2020年给药时机合理率均为100%;2017年与各年度给药疗程之间比较差异无统计学意义(P>0.05);2020年品种选择较其他年份好(均为头孢唑啉),2017年与2020年对比,在品种选择合理率方面差异具有统计学意义(P<0.01)。结论某院甲状腺手术围术期预防性应用抗菌药物使用比较规范,但仍需持续监测和督导。Objective To study the effect of driver's license scoring management on perioperative prophylactic use of antibiotics during thyroid surgery in a hospital from 2017 to 2020.Methods A total of 503 cases were included,and the data of age,sex,diagnosis,selection of antimicrobial drug variety,selection of administration time and time of use were recorded and analyzed.Results In 2017(before driver's license management),the utilization rate of antibiotics was higher than 30%(31.78%),and lower than the national requirement of<30%in other years;the utilization rate of antimicrobial prophylaxis in 2017 was compared with that in other years,and there was statistical significance(P<0.01).In 2017,the rational rate of preoperative administration time was only 55.88%,the rational rate of drug administration timing was 100%from 2018 to 2020;there was no difference in the treatment duration between 2017 and other years(P>0.05).The variety selection in 2020 was better than that in other years(all of them were cefazolin),and the reasonability rate of variety selection in 2017 was statistically significant compared with 2020(P<0.01).Conclusion Perioperative prophylactic use of antibiotics in thyroid surgery in the hospital is standard,but it still needs continuous monitoring and supervision.
