基于数据挖掘技术研究针灸治疗围绝经期失眠症的选穴规律

摘　　要：目的:通过数据挖掘技术探究针灸治疗围绝经期失眠症的临床取穴规律。方法:检索国家知识基础设施数据库(CNKI)、中国学术期刊数据库(CSPD)、中文科技期刊数据库(CCD)、中国生物医学文献数据库(CBM)、PubMed、Web of Science、Cochrane Library、Embased自建库至2021年11月公开发表的针刺治疗围绝经期失眠症的有关文献,通过纳入、排除标准进行文献筛选,建立针灸处方数据库。对获得数据运用Excel表格进行频次统计分析,SPSS molder进行聚类和关联规则分析。结果:针灸治疗围绝经期失眠症腧穴使用频次较高的是三阴交、百会、神门、四神聪、肾俞、太冲等。经络选用以足太阳经、督脉为主。所选腧穴主要位于下肢部,其次是头颈部。特定穴中交会穴、原穴、五输穴、背俞穴使用较多。关联规则分析显示气海-关元、肾俞-肝俞、心俞-肾俞相关性较高,对使用频次6次以上腧穴进行聚类分析,得到7个聚类群。结论:针灸治疗围绝经期失眠症疗效确切,在辨病取穴的基础上,更注重调补脏腑、安神定志,充分体现了中医整体观念的医学特色。Objective:To use data mining technology to explore clinical acupoint selection rules for menopausal insomnia.Methods:China National Knowledge Infrastructure(CNKI),China Science Periodical Database(CSPD),Chinese Citation Database(CCD),China Biology Literature Medicine(CBM),PubMed,Web of Science,Cochrane Library,andEmbased were searched from their establishment to November 2021,and the published literature on acupuncture treatment of menopausal insomnia was screened by inclusion and exclusion criteria,with a database of acupuncture prescriptions established.Excel was used to perform frequency statistical analysis on the obtained data,and SPSS molder was adopted to perform clustering and association rule analysis.Results:In acupuncture treatment of menopausal insomnia,the most frequantly used acupoints are Sanyinjiao(SP 6),Baihui(GV 20),Shenmen(HT 7),Sishencong(EX-HN 1),Shenshu(BL 23),and Taixi(LR 3).The main meridians are the musculature bladder meridian and the governor channel.The acupoints are located in the lower limbs,followed by the head and neck.Among the specific acupoints,Jiaohui,Yuan,Wushu,and Beishu are more frequently used.Association rule analysis shows that Qihai(CV 6)-Guanyuan(CV 4),Shenshu(CV 4)-Ganshu(BL 18),and Xinshu(BL 15)-Shenshu(CV 4)are highly correlated.Cluster analysis of acupoints used more than six times results in seven cluster groups.Conclusion:Acupuncture is effective in treating menopausal insomnia.On the basis of disease differentiation and acupoint selection,more attention should be paid to regulating and supplementing the zang-fu organs and calming the mind and settling the mind,which fully embodies the medical characteristics of the overall concept of Traditional Chinese medicine.
