摘 要:目的 探讨糖化血红蛋白(glycosylated hemoglobin type A1c, HbA1c)与空腹血糖(fasting plasma glucose, FPG)在糖尿病患者中的诊断效能。方法 选取2019年1月—2021年12月厦门大学附属第一医院门诊收治的100例糖尿病患者为观察组;选择同期100例健康体检者为对照组。采用全自动生化全血分析仪检测两组FPG;采用全自动高压液相分析测定两组HbA1c;绘制受试者工作(receiver operator characteristic, ROC)曲线,分析HbA1c和FPG在糖尿病患者中的诊断效能(灵敏度和特异度)。结果 观察组HbA1c和FPG水平高于对照组,差异有统计学意义(P<0.05);ROC曲线显示,HbA1c和FPG联合检测的诊断灵敏度、特异度高于单一检测HbA1c或FPG,差异有统计学意义(P<0.05)。结论 糖尿病患者的HbA1c和FPG水平异常,采用HbA1c和FPG联合检测可提升诊断灵敏度、特异度,临床在诊疗糖尿病过程中可将其作为一定的依据。Objective To investigate the diagnostic efficacy of glycosylated hemoglobin type A1c(HbA1c)combined with fasting plasma glucose(FPG)in diabetic patients.Methods A total of 100 diabetic patients admitted to the outpatient department of the First Affiliated Hospital of Xiamen University from January 2019 to December 2021 were selected as the observation group.100 people under physical examination in the same period were selected as the control group.FPG was measured by automatic biochemical whole blood analyzer,and HbA1c was measured by automatic high-pressure liquid phase analysis.Receiver operator characteristic(ROC)curves were drawn to analyze the diagnostic efficacy(sensitivity and specificity)of HbA1c and FPG in patients with diabetes.Results The levels of HbA1c and FPG in diabetic patients in observation group were higher than those in control group,the difference was statistically significant(P<0.05).According to the ROC curve,the sensitivity and specificity of the combined detection of HbA1c and FPG for the diagnosis of diabetic patients were higher than those of the single detection of HbA1c or fasting plasma glucose,the differences were statistically significant(P<0.05).Conclusion The level of HbA1c and FPG is abnormal in patients with diabetes,and the combined detection of HbAlc and FPG can improve the diagnostic sensitivity and specificity,which can be used as a certain basis in the clinical diagnosis and treatment of diabetes.
