摘 要:目的 分析妊娠糖尿病实施助产士孕期营养指导的影响。方法 选取2022年9月—2023年9月临沂市中医医院收治的98例妊娠糖尿病患者为研究对象,按随机数表法分为营养组和一般组,各49例。营养组行助产士孕期营养指导,一般组行孕期常规指导,检测两组患者血糖指标,观察营养情况,比较母婴结局。结果 指导前,两组血糖指标比较,差异无统计学意义(P>0.05);指导后,营养组血糖指标均低于一般组,差异有统计学意义(P<0.05)。指导前,两组患者白蛋白以及前白蛋白指标水平比较,差异无统计学意义(P>0.05),指导后,营养组患者的白蛋白以及前白蛋白指标均高于一般组,差异有统计学意义(P<0.05)。营养组的不良产妇结局发生率为4.08%,低于一般组的16.33%,差异有统计学意义(P<0.05)。营养组的不良新生儿结局发生率为6.12%,低于一般组的22.45%,差异有统计学意义(P<0.05)。结论 助产士孕期营养指导护理可产生正面影响,减少不良母婴结局,还能改善营养状态,降低血糖,可用于妊娠糖尿病群体的护理中。Objective To analyze the impact of implementing midwives'nutritional guidance during pregnancy in gestational diabetes mellitus.Methods A total of 98 patients with gestational diabetes mellitus admitted to Linyi Hospital of Traditional Chinese Medicine from September 2022 to September 2023 were selected as the research objects and grouped according to random number table method into nutritional group and general group,both with 49 cases.The nutritional group performed midwives'nutritional guidance during pregnancy,and the general group performed routine guidance during pregnancy.Blood glucose indicators were measured,nutritional status was observed,and outcomes of mathers and infants were compared in both groups of patients.Results Before guidance,there was no statistically significant difference in blood glucose indicators between the two groups of patients(P>0.05).The blood glucose indicators after guidance were lower in the nutritional group than in the general group,and the differences were statistically significant(P<0.05).Before guidance,there was no statistically significant difference in albumin and prealbumin indicators between the two groups of patients(P>0.05).After guidance,the albumin and prealbumin indicators of the nutrition group were higher than those of the general group,and the differences were statistically significant(P<0.05).The incidence of adverse maternal outcomes in the nutrition group was 4.08%,lower than the general group's 16.33%,and the difference was statistically significant(P<0.05).The incidence of adverse neonatal outcomes in the nutrition group was 6.12%,lower than the 22.45%in the general group,and the difference was statistically significant(P<0.05).Conclusion Midwives'nutritional guidance care during pregnancy can have a positive impact on reducing adverse maternal and infant outcomes,and can also improve nutritional status and lower blood glucose,which can be used in the care of gestational diabetes mellitus group.
