摘　　要：报告2例Fournier坏疽。例1.男,30岁。阴囊、阴茎肿大伴破溃、发热8 d。皮肤科检查:阴茎及阴囊皮肤多处大面积破溃、坏死,最大4 cm×10 cm、表面有黑痂,周围弥漫性肿胀伴渗液,其上可触及捻发音,流出大量尸臭味脓性分泌物。例2.男,40岁。下腹部疼痛伴阴囊肿胀7 d。皮肤科检查:阴囊、阴茎皮肤大片坏死、破溃,最大为6 cm×12 cm,挤压双侧腹股沟区有恶臭味淡黄色脓液流出。2例患者经及时应用广谱抗生素和急诊手术彻底清创引流及理疗等对症治疗。例1患者因脓毒血症死亡,例2患者术中清创面积扩大,择期行皮瓣移植。Two cases of Fournier's gangrene are reported.Case 1.A 30-years-old male presented with swollen penis and scrotum and fever for eight days.Dermatological examination showed multiple large necrotic areas and ulceration in the skin of the penis and scrotum,the largest of which measured 4 cm x 10 cm.Diffuse swelling of the surrounding tissues,and a large amount of foul smelling purulent exudate were also noticed.Case 2.A 40-years-old male presented with abdominal pain and scrotal swelling for seven days.Dermatological examination showed large necrotic scab of scrotal and penile skin,areas of ulceration(the largest being about 6 cm x 12 cm).Foul smelling yellowish pus could be drained from the inguinal area bilaterally.Both patients were put on broad-spectrum antibiotics,emergency surgery,thorough debridement and drainage,and physical therapy.Case 1 died of sepsis.Case 2,due to large area of debridement during operation,needed a second intervention,reconstructive plastic surgery with local fasciocutaneous flap.
