富血小板血浆联合负压封闭引流对慢性难愈性创面的影响

摘　　要：目的 研究富血小板血浆(PRP)联合负压封闭引流(VSD)对慢性难愈性创面(CRW)愈合的影响。方法 选择2018年12月~2021年3月在浙江省丽水市人民医院接受治疗的CRW患者88例,按照随机数字表法进行分组,VSD组44例单用VSD治疗,VSD联合PRP组44例在VSD治疗的基础上联合PRP治疗。比较两组创面愈合情况、疼痛程度、氧化应激及血管新生情况。结果 VSD联合PRP组治疗14天创面愈合率(84.09%)较VSD组(63.64%)高,而创面愈合时间[(32.52±3.15)天]较VSD组[(38.65±3.26)天]短,差异有统计学意义(P<0.05或P<0.01)。两组治疗14天后VAS评分均降低,且VSD联合PRP组较低,差异有统计学意义(P<0.05或P<0.01)。两组治疗14天后MDA、AOPP均降低,且VSD联合PRP组更低,而SOD升高,且VSD联合PRP组更高,差异有统计学意义(P<0.05或P<0.01)。两组治疗14天后bFGF、VEGF均升高,且VSD联合PRP组更高,而ES降低,且VSD联合PRP组更低,差异有统计学意义(P<0.05或P<0.01)。结论 PRP联合VSD治疗CRW疗效确切,可有效缩短创面愈合时间,减轻痛苦,抑制氧化应激,促进血管新生。Objective To investigate the effect of platelet-rich plasma(PRP)combined with negative pressure closed drainage(VSD)on the healing of chronic refractory wound(CRW).Methods A total of 88 patients with CRW who received treatment in Zhejiang Lishui People’s Hospital from December 2018 to March 2021 were divided into groups according to the random number table,44 patients in the control group were treated with VSD alone,and 44 patients in the study group were treated with PRP on the basis of VSD.Comparative analysis of wound healing,degree of pain,oxidative stress,and angiogenesis.Results The wound healing rate(84.09%)in the study group after 14 days of treatment was higher than that in the control group(63.64%),while the wound healing time[(32.52±3.15)days]was shorter than that in the control group[(38.65±3.26)days](P<0.05 or P<0.01).VAS scores in both groups were significantly decreased after 14 days of treatment,and the study group was lower(P<0.05 or P<0.01).After 14 days of treatment,MDA and AOPP were significantly decreased in both groups,and lower in the study group,while SOD was significantly increased,and higher in the study group(P<0.05 or P<0.01).After 14 days of treatment,bFGF and VEGF were significantly increased in both groups and higher in the study group,while ES was significantly decreased and lower in the study group(P<0.05 or P<0.01).Conclusion PRP combined with VSD is effective in the treatment of CRW,which can effectively shorten wound healing time,relieve pain,inhibit oxidative stress and promote angiogenesis.
