左炔诺孕酮宫内缓释系统与地屈孕酮治疗子宫内膜增生不伴不典型增生的临床疗效分析

摘　　要：目的探讨子宫内膜增生不伴不典型增生采用左炔诺孕酮宫内缓释系统与地屈孕酮治疗的效果。方法选取60例子宫内膜增生不伴不典型增生患者为研究对象,根据随机数字表法分为对照组和研究组,每组30例。对照组给予地屈孕酮治疗,研究组给予左炔诺孕酮宫内缓释系统治疗,比较两组子宫内膜厚度、月经持续时间、不良反应发生率、治疗效果及半年后病理检查阳性率。结果治疗前,两组子宫内膜厚度、月经持续时间比较,差异均无统计学意义(P>0.05);治疗后,研究组子宫内膜厚度优于对照组,月经持续时间短于对照组(P<0.05);研究组不良反应发生率低于对照组(P<0.05);治疗后,研究组总有效率高于对照组(P<0.05);研究组半年后病理检查阳性率低于对照组(P<0.05)。结论子宫内膜增生不伴不典型增生采用左炔诺孕酮宫内缓释系统治疗效果较好,可改善子宫内膜厚度,缩短月经持续时间,减少不良反应发生率及半年后病理检查阳性率,值得临床推广与应用。Objective To investigate the effect of levonorgestrel intrauterine sustained-release system and dydrogestrel in the treatment of endometrial hyperplasia without atypical hyperplasia.Methods A total of 60 patients with endometrial hyperplasia without atypical hyperplasia were selected as study objects and divided into control group and study group according to random number table method,with 30 cases in each group.The control group was treated with dydrogesterone,and the study group was treated with levonorgestrel intrauterine sustained release system.Endometrial thickness,menstrual duration,incidence of adverse reactions,therapeutic effect and positive rate of pathological examination six months later were compared between the two groups.Results Before treatment,there were no significant differences in endometrial thickness and menstrual duration between the two groups(P>0.05).After treatment,endometrial thickness in the study group was better than that in the control group,and menstrual duration was shorter than that in the control group(P<0.05).The incidence of adverse reactions in study group was lower than that in control group(P<0.05).After treatment,the total effective rate of the study group was higher than that of the control group(P<0.05).The positive rate of pathological examination in the study group was lower than that in the control group six months later(P<0.05).Conclusion Levonorgestrel intrauterine sustained release system is effective in the treatment of endometrial hyperplasia without atypical hyperplasia,which can improve endometrial thickness,shorten menstrual duration,reduce the incidence of adverse reactions and the positive rate of pathological examination six months later,and is worthy of clinical promotion and application.
