子宫动脉预置管联合动脉栓塞术在中央性前置胎盘剖宫产术中的应用研究

摘　　要：目的探讨在中央性前置胎盘剖宫产手术中子宫动脉预置管联合动脉栓塞术的效果。方法选取40例行剖宫产分娩的中央性前置胎盘产妇,其中20例剖宫产术前行子宫动脉预置管联合动脉栓塞术产妇作为观察组,20例仅行剖宫产术产妇作为对照组。对比两组应用效果。结果观察组术中出血量、术中输血量、产后出血和输血情况与对照组比较,差异有统计学意义(P<0.05)。两组的手术前后血红蛋白水平、手术时间和术后住院时间比较,差异无统计学意义(P>0.05)。观察组新生儿体重、1min的Apgar评分、5min的Apgar评分分别为(2844.0±191.6)g、(9.4±1.2)分、(9.9±0.5)分;对照组新生儿体重、1min的Apgar评分、5min的Apgar评分分别为(2750.0±222.8)g、(9.8±0.4)分、(9.9±0.4)分。两组新生儿体重、1min的Apgar评分、5min的Apgar评分比较,差异无统计学意义(P>0.05)。结论子宫动脉预置管联合动脉栓塞术可以有效减少剖宫产手术中出血量和输血量,有效降低产后出血发生率,同时对新生儿的影响较小,具有临床应用价值。Objective To investigate the effect of uterine artery prepositioning combined with arterial embolization in central placenta previa cesarean section.Methods 40 women with central placenta previa who were delivered by cesarean section were selected,of which 20 were selected as the observation group and 20 were selected as the control group.The application effect of the two groups was compared.Results The intraoperative blood loss,intraoperative blood transfusion,postpartum hemorrhage and blood transfusion in the observation group were significantly different from those in the control group(P<0.05).There was no significant difference in hemoglobin level,operation time and postoperative hospital stay between the two groups(P>0.05).The weight,Apgar score of 1 min and Apgar score of 5 min were(2844.0±191.6)g,(9.4±1.2)points and(9.9±0.5)points,respectively.The weight,Apgar score of 1 min and Apgar score of 5 min in the control group were(2750.0±222.8)g,(9.8±0.4)points and(9.9±0.4)points,respectively.There was no significant difference in body weight,Apgar score of 1 min and Apgar score of 5 min between the two groups(P>0.05).Conclusion Uterine artery prepositioning combined with arterial embolization can effectively reduce the amount of blood loss and blood transfusion during cesarean section operation,effectively reduce the incidence of postpartum hemorrhage,and have little impact on newborns,which has clinical application value.
