子宫局部压迫缝合术联合子宫动脉上行支结扎术治疗前置胎盘产后出血的疗效评价

摘　　要：目的评估子宫局部压迫缝合术(ULCS)+子宫动脉上行支结扎术应用在前置胎盘产后出血治疗中的临床疗效。方法选取68例前置胎盘产后出血产妇,参照随机数字表法分为对照组(采用ULCS治疗)与试验组(采用ULCS+子宫动脉上行支结扎术治疗),每组34例。对比两组出血量、止血效果及并发症发生情况。结果相比对照组,试验组各时间点(术中、术后2h、12h、24h)的出血量更少(P<0.05)。相比对照组(82.35%),试验组总止血率(97.06%)更高(P<0.05)。相比对照组(23.53%),试验组并发症发生率(5.88%)更低(P<0.05)。结论在前置胎盘产后出血中采用ULCS+子宫动脉上行支结扎术,可以增强止血效果,减少出血量及并发症发生率,值得临床推广。Objective TTo evaluate the clinical efficacy of uterine local compression suture(ULCS)and ligation of the ascending branch of uterine artery in the treatment of postpartum hemorrhage of placenta previa.Methods 68 women with placenta previa postpartum hemorrhage were selected and divided into control group(ULCS treatment)and experimental group(ULCS+ascending uterine artery ligation treatment)according to random number table method,with 34 cases in each group.The amount of blood loss,hemostatic effect and complications were compared between the two groups.Results Compared with the control group,the blood loss of experimental group was less at each time point(intraoperative,2 h,12 h,24 h)(P<0.05).Compared with the control group(82.35%),the total hemostasis rate of experimental group(97.06%)was higher(P<0.05).Compared with the control group(23.53%),the complication rate of experimental group(5.88%)was lower(P<0.05).Conclusion The use of ULCS+ligation of ascending uterine artery in placenta previa postpartum hemorrhage can enhance the hemostatic effect,reduce the amount of bleeding and the incidence of complications,and is worthy of clinical promotion.
