妊娠期高血压综合征产妇实施全产程分娩镇痛对其产程的影响

摘　　要：目的分析妊娠期高血压综合征(PIH)产妇实施全产程分娩镇痛对产程的影响。方法选取60例PIH产妇,根据随机数字表法分为两组,每组30例。甲组给予全产程分娩镇痛,乙组给予第一产程分娩镇痛。对比两组的产程时间以及分娩结局。结果甲组产妇在阴道试产中转剖宫产率为13.33%,乙组为16.67%,比较差异无统计学意义(P>0.05)。两组成功阴道分娩产妇的第一产程时间、第二产程时间以及第三产程时间比较差异无统计学意义(P>0.05)。甲组在第二产程屏气用力时(t_(3))和胎头娩出时(t_(4))的视觉模拟评分法(VAS)评分均低于乙组(P<0.05);两组的产后出血发生率以及新生儿娩出后1 min、5 min的Apgar评分比较,差异无统计学意义(P>0.05)。结论全产程分娩镇痛在PIH产妇中应用安全可靠,且对产程无明显不良影响,对分娩结局的影响也较小,还能有效缓解产妇的产痛。Objective To analyze the effect of full labor analgesia in pregnant women with pregnancy-induced hypertension syndrome(PIH).Methods 60 PIH parturients were selected and divided into two groups according to random number table method,with 30 cases in each group.Group A was given full labor analgesia,group B was given first labor analgesia.The labor duration and delivery outcome of the two groups were compared.Results The rate of cesarean section delivery was 13.33%in group A and 16.67%in group B,and the difference was not statistically significant(P>0.05).There was no significant difference in the first stage of labor,second stage of labor and third stage of labor between the two groups(P>0.05).The VAS scores of group A during the second stage oflabor(t_(3))and fetal head delivery(t_(4))were lower than those of group B(P<0.05).There was no significant difference in the incidence of postpartum hemorrhage and Apgar score 1 min and 5 min after delivery between the two groups(P>0.05).Conclusion Full labor analgesia is safe and reliable in PIH parturients,and has no obvious adverse effects on labor and labor outcome,but it can effectively relieve parturients'labor pain.
