老年下肢深静脉血栓患者血清Gas6、IL-1β、NETs水平与介入治疗效果的关系

摘　　要：目的探讨老年下肢深静脉血栓(DVT)患者血清生长停滞特异性基因产物6(Gas6)、白细胞介素-1β(IL-1β)、中性粒细胞胞外陷阱(NETs)水平与介入治疗效果的关系。方法选取2021年2月至2023年1月该院收治的120例老年DVT患者为研究对象,根据治疗7 d后临床疗效分为无效组(22例)和有效组(98例)。比较两组治疗前及治疗3、7 d后血清Gas6、IL-1β、NETs水平,分析治疗3、7 d后血清Gas6、IL-1β、NETs水平与治疗效果的关系。结果无效组与有效组Gas6、IL-1β、NETs水平存在时间与组间的交互效应,差异有统计学意义(F交互=15.214,P<0.001);两组Gas6、IL-1β、NETs水平随着治疗时间变长而逐渐降低,差异均有统计学意义(F=12.375、12.271、13.277,P<0.001);无效组Gas6、IL-1β、NETs水平明显高于有效组,差异均有统计学意义(F=23.537、22.851、23.213,P<0.001)。治疗3、7 d后血清Gas6、IL-1β、NETs联合检测预测DVT患者介入治疗无效的曲线下面积明显大于各项指标单独检测(P<0.05)。结论血清Gas6、IL-1β、NETs水平对预后有明显影响,3项指标联合检测可作为预测DVT患者介入治疗效果的重要辅助途径。Objective To investigate the correlations between the levels of serum growth arrest-specific gene product 6(Gas6),interleukin-1β(IL-1β),neutrophil extracellular trap(NETs)and interventional therapy in elderly patients with lower extremity deep vein thrombosis(DVT).Methods A total of 120 elderly patients with DVT admitted to Xuchang Hospital from February 2021 to January 2023 were divided into ineffective group(22 cases)and effective group(98 cases)according to the clinical efficacy after 7 days of treatment.To compare the levels of serum Gas6,IL-1β and NETs before treatment and after 3 and 7 d of treatment between the two groups,and to analyze the relationships between the levels of serum Gas6,IL-1β,NETs after 3 and 7 d of treatment and the treatment effect.Results The interaction effects of Gas6,IL-1β and NETs between the ineffective group and the effective group were statistically significant(F interaction=15.214,P<0.001).The levels of Gas6,IL-1βand NETs in the two groups gradually decreased with the increase of treatment time,the differences were statistically significant(F=12.375,12.271,13.277,P<0.001).The levels of Gas6,IL-1β and NETs in the ineffective group were significantly higher than those in the effective group,and the differences were statistically significant(F=23.537,22.851,23.213,P<0.001).After 3 and 7 days of treatment,the combined detection of serum Gas6,IL-1β and NETs predicted the ineffective interventional therapy in DVT patients with the area under the curve was significantly greater than that of each index alone(P<0.05).Conclusion Serum Gas6,IL-1β and NETs levels have significant effects on prognosis,and combined detection of the three indicators can be used as an important auxiliary way to predict the effect of interventional therapy in DVT patients.
